What Is Rectal Prolapse?

Rectal prolapse is when the rectum -the last section of the large intestine- drops down or slides out of the anus. It is typically not considered a medical emergency. Rectal prolapse can sometimes be treated with suppositories, stool softeners, and other medications. But surgery is usually required to treat rectal prolapse.

There are three types of rectal prolapse:

  • External prolapse: The entire rectum drops out of the anus.
  • Mucosal prolapse: Part of the rectal lining pokes out of the anus.
  • Internal prolapse: The rectum has started to drop but is not yet poking out of the anus.

    Rectal Prolapse Diagnosis And Surgery

    Diagnosis

The diagnosis of rectal prolapse begins with a physical examination. In the physical examination, the doctor examines the rectum and may insert a lubricated and gloved finger in the anus to check the strength of the anal sphincter muscles and the rectum. Also, the doctor will ask you to describe your medical history and symptoms to understand your condition better. They can even ask you to squat and strain as you do during the bowel movement.

Based on the physical assessment findings, the doctor can ask you for other tests to diagnose the medical condition better. These tests include:

    1. Anal electromyography (EMG)– Anal sphincter electromyography (EMG) is recorded using a small sponge electrode in the anal canal. This test measures how effective the muscles and nerves in the rectum are.
    2. Anorectal manometry- A flexible, thermometer-sized tube with a small balloon attached to the end is inserted into the rectum. While connected to a machine, this tube is placed in different areas of the rectum, and the response to pressure is estimated. This test helps measure the tightness of the anal sphincter and the functioning of the rectum.
    3. Barium enema– A chalky liquid having barium is placed in the rectum during this test. Barium shows up on X-rays producing images of the area.
    4. Defecography– It is a radiology test to look inside at the muscles and organs while you defecate. Defecography is used to help determine the cause of constipation, fecal incontinence, and other medical conditions like pelvic organ prolapse that may interfere with a person’s ability to pass stool.
    5. Colonoscopy– In this test, a thin, flexible tube with a tiny camera at the end is inserted into the rectum so the doctor can do a visual exam.
    6. Transit study– This test demonstrates how well food moves through the colon. The patient swallows one or more capsules containing small markers that are visible on the X-ray. The patient follows a high-fiber diet during this test, and the movement of the markers through the colon is observed with abdominal X-rays taken several times after the capsule is swallowed.
    7. Other exams– Your doctor may want to perform other tests to see if there are any weak areas in the pelvic floor or if other organs, like the uterus, have prolapsed.

    Surgery

There are two kinds of rectal prolapse surgeries:

    • Rectopexy or rectal prolapse repair through the abdomen- In rectopexy, an incision is made in the abdomen, and the surgeon pulls the rectum back in place. Using a mesh sling or sutures, they anchor the rectum to the back wall of the pelvis, known as the sacrum. In some cases, like a long history of constipation, the surgeon removes a portion of the colon.
    • Laparoscopic rectal prolapse surgery- Laparoscopic surgery for rectal prolapse is also done through the abdomen through several smaller incisions. The surgeon inserts surgical tools and a tiny camera through the abdominal incisions to repair the rectal prolapse.

Some major factors that determine the cost of surgery include the location of the hospital where the surgery takes place, the healthcare center, hospital accommodation charges, the surgeon’s fees, the patient’s age, the severity of the condition, and more.

    Can a rectal prolapse burst?

Unforced rupture of the rectum due to increased intra-abdominal pressure is an infrequent complication of rectal prolapse in adults.

When to call the healthcare provider for rectal prolapse treatment?

Call your healthcare provider if you experience any of these symptoms along with a prolapsed rectum:

    • Fever
    • Bleeding
    • Discharge
    • Chills
    • Redness
    • Swelling
    • Constipation
    • Fecal incontinence

What is the best medicine for rectal prolapse?

There is no specific medicine for rectal prolapse. However, stool softeners can help decrease bowel movement straining caused by constipation.

Does drinking water help rectal prolapse?

Drinking plenty of water and eating fruits, vegetables, and other foods that contain fiber can help treat rectal prolapse. Changes in diet are enough to improve or reverse a prolapse of the rectum’s lining (partial prolapse). Furthermore, doing kegel exercises can also help strengthen the pelvic area muscles.

    Do hemorrhoids turn into prolapse?

Hemorrhoids can become prolapsed when the tissue that holds them in place weakens. Several possible causes and risk factors may contribute to the weakening of the connective tissue. Straining during bowel movements is one such cause as it puts extra pressure on hemorrhoids.

    How serious is rectal prolapse?

While rectal prolapse is not a medical emergency, it can aggravate if left untreated. Some complications that can occur include bleeding, ulceration, and a reduction in the blood supply that can result in gangrene or death of the affected tissue and cause strangulation of the rectum.

Can you reverse a rectal prolapse naturally?

In some cases, it is possible to ease symptoms or reverse a mild rectal prolapse by doing pelvic muscle exercises and self-care measures. But rectal prolapse surgery is the only way to properly treat rectal prolapse and relieve symptoms.

How To Prepare For Rectal Prolapse Surgery?

To prepare for the surgery, your doctor may ask you to:

    • Clean with a special soap- Before the surgery for rectal prolapse, your doctor can ask you to take a shower using antiseptic soap to help prevent germs on the skin from causing infection after the surgery.
