    Diagnosis and Management

    Diagnostic Tests

Generally, physical examination is the biggest determinant for circumcision surgery. Many patients get circumcision as they dislike the shape, size, color, or texture of their foreskin. Moreover, patients with foreskin-related issues like phimosis and balanitis have a swollen, reddish foreskin with blood and discharge in some cases. In case the patient has an infected foreskin, the doctor may conduct a swab test to identify the cause of the infection.

    Surgery

Once the diagnosis has been made and the patient has made up their mind regarding circumcision surgery, the doctor will draft a treatment plan. If medical treatment is possible, the patient will be treated accordingly, but if medical management is not possible, the doctor will suggest the appropriate surgical intervention.
Different techniques for circumcision surgery are:

    • Laser circumcision: Laser circumcision is the safest and most preferred treatment for most patients. During the surgery, the surgeon uses a laser beam to cut off the foreskin. Since a laser is used, there is no bleeding and no sutures are required.
    • Stapler circumcision: Stapler circumcision is performed using a specialized stapler device called an Anastomat. The stapler is fit on the foreskin and pulled. It removes the foreskin in a quick motion and replaces it with a silicone ring. The silicone ring falls off on its own in 7-10 days after the incision has healed.

how much does it cost to get circumcision for adults in Dhaka?

On average, the cost of circumcision surgery for adults in Dhaka ranges from ৳ 30000 to ৳ 35000. However, this depends on the reason for circumcision and the medical condition of the patient (penile foreskin-related issues like phimosis, paraphimosis, posthitis, etc. ).

How long does it take to recover from circumcision surgery?

Depending on the type of surgery and the severity of the condition, it can take 7-10 days for the patient to recover completely. For conventional open surgery, the patient may need downtime of about 2-3 days but for advanced procedures like laser and stapler circumcision, no downtime is required.

Can circumcision affect fertility or semen production?

No, circumcision is performed at the tip of the penis, while semen and testosterone production occurs in the scrotum As such, it does not affect fertility or semen production at all.

Can I have sex after circumcision?

Yes, you can have sex after circumcision, but most surgeons recommend waiting at least 4-5 days before engaging in any sexual activity to allow the patient enough time to heal. If you have sex before this, you can irritate the incisions leading to further injury.

Can circumcision be done at any age?

Yes, as long as the patient is healthy, they can safely undergo circumcision at any age. Traditionally, circumcision is performed at birth or within the child’s first month of life, but medical circumcision can be performed at any age.

Will circumcision permanently treat foreskin problems?

Barring any complications, circumcision completely removes the foreskin to treat foreskin issues. In some cases, if the foreskin is not completely removed or if it’s torn, it can grow back.

Which foreskin/penile conditions can be treated via circumcision surgery?

Adult circumcision is performed as a treatment for the following conditions:

    • Phimosis: In phimosis, the foreskin gets stuck and can’t be pulled back from the tip of the penis without considerable pain and discomfort.
    • Paraphimosis: Paraphimosis is a medical emergency in which the retracted foreskin gets stuck behind the penis tip and can’t be pulled back. It can restrict blood flow to the penis and lead to tissue damage.
    • Posthitis: Posthitis is the inflammation of the foreskin due to poor hygiene, allergy, and bacterial or fungal infections.
    • Balanitis: Balanitis is pain and inflammation of the penis glans (head of the penis) that may happen due to trauma, infection, or poor hygiene.
    • Balanoposthitis: Balanoposthitis occurs when both the glans penis and prepuce get infected. It usually occurs due to yeast infections, irritants, trauma, etc.
    • Preputial pearls: Preputial pearls or cysts are formed when smegma gets collected under the foreskin. Smegma is a buildup of dead skin cells, oil, and other fluids that appears as a thick, white, cheesy substance under the foreskin.
    • Redundant foreskin: Redundant foreskin or prepuce is a condition in which the patient has an excess amount of foreskin which completely covers the head of the penis when it is not erect.

How can you improve recovery after circumcision surgery?

Here are a few tips to improve your recovery after circumcision:

    • Hydrate yourself well by drinking at least 7-8 glasses of water every day. You should also include other drinks like coconut water, electrolytes, etc. it will increase the pH of your urine and reduce its acidity. This will help reduce pain and irritation during urination and help recovery.
    • Rather than wearing loose underwear or boxers, wear tight underwear that will hold your penis in position. If you have a dressing on your penis, loose underwear can lead to the dressing coming off of the penis.
    • Refrain from strenuous physical activities like running, lifting heavy weights, cycling, etc., until the wound is healed completely. Stress on the healing wound can lead to swelling, reopening of the incision, and delay in recovery.
    • Take your medicines as prescribed by your urologist. Skipping medicines can increase the risk of infections. Medicinal creams also reduce the risk of scarring on the penis.
    • Rub vaseline or petroleum jelly on the tip of the penis to get relief from the stinging or burning sensation while urinating.
If you have unbearable pain or uncontrolled bleeding at any time during your recovery, you should connect with your doctor immediately.
    • Keep the surgical area clean and dry. You can use a damp sponge or cloth to clean your penile area. Wait at least a couple of days to take a full body bath. Change the dressing regularly as directed by your urologist.
    • Wait at least a couple of weeks before engaging in any sexual activity or masturbation to avoid irritating the healing injury.
    • Avoid greasy and junk foods that can lead to constipation. Add fibrous and nutrition-rich food to your diet to aid digestion. Pressure due to constipation can irritate the healing incision and lead to complications.
