What is Breast Augmentation and Its Types?

Breast augmentation is a surgical treatment performed to increase the breast size in women. In general, breasts are considered one of the most integral parts of a woman’s femininity, and it affects her self-esteem. Though the main purpose of breast augmentation surgery is to enlarge breasts, it can also help to restore the lost volume, improve breast symmetry, and reconstruct breasts after mastectomy or lumpectomy procedures.

There are two types of breast augmentation treatment-

Fat Transfer for Augmentation: This type of breast augmentation involves extracting fat tissues from a specific area of the body, such as hips, flanks, stomach, or thighs, using liposuction and re-injecting it into the breasts after filtration. Fat transfer breast augmentation is done when the patient wishes to add a small volume to the breasts, usually up to 1 cup size. This method is also suitable to make the breasts symmetrical and add cleavage.
Breast Implants: This type of augmentation is the gold standard to increase breast size. In this type, particular types of prosthetic devices called breast implants, are inserted into the chest pocket to enhance the size and create a more symmetrical and contoured breast profile. Almost 90% of breast augmentation surgeries are performed using breast implants. Due to this, the treatment is synonymously called breast implant surgery.

Diagnosis and Evaluations Done Before Breast Augmentation

During the initial consultations, the plastic surgeon tries to understand the woman’s concerns regarding her breasts and also evaluates her physical and psychological state. The patient’s psychological state helps to determine her motivations for the surgery and whether she is emotionally stable and has realistic expectations.

The surgeon asks about the desired goals, medical history, previous surgeries, allergies, and the intake of prescribed and over-the-counter medications. Along with this, the surgeon also palpates the breast tissues to examine the size, shape, and texture of the breast tissues and the skin quality. The accurate measurements of the breast size, the nipple’s location, areola diameter, nipple-to-breast crease distance, etc., are taken to create a personalized treatment plan.

In some cases, the surgeon may recommend imaging tests, such as a mammogram or ultrasound, to examine the breast tissues. How to Prepare for Breast Augmentation Surgery?

Before undergoing any type of surgery, the patient is given some preparation tips to mitigate the potential risks and complications. By following the general instructions given by the doctor, the patient can ensure that the surgery goes smoothly. The first thing that the patient needs to do is get the recommended tests done right away, including a complete blood test, liver function, kidney function, etc. These tests help to determine whether the patient is in optimal health to undergo the surgery or needs to make certain lifestyle changes.

Based on the initial health evaluations, the doctor may provide the following instructions:

2 Weeks Before the Procedure

The patient is advised to start preparing for breast augmentation surgery at least two weeks before the procedure is scheduled. This includes making the necessary arrangements for support or help with house chores, childcare, and work-related responsibilities.
The patient needs to stop drinking alcohol at least two weeks before the surgery. Alcohol causes extreme dehydration and also acts as a blood thinner, which can impact the body‘s healing abilities.
Similarly, the patient also needs to stop smoking entirely. Smoking severely affects the blood vessels and impairs their function, which can increase the risk of post-surgical complications.
The doctor will adjust the patient’s medication and advise avoiding medicines that can increase the risk of bleeding or blood clots. This includes avoiding taking aspirin, ibuprofen, and other herbal supplements that can result in blood-related disorders.
Start eating a healthy and nutritious diet, as the body needs them for optimal healing. If the patient is on a diet, she will be asked to stop and eat a balanced diet rich in vitamins, minerals, carbohydrates, etc.

1 Week Before the Surgery

At this point, the doctor’s team will advise the patient to arrange the recovery area and get some essential supplies that will be needed during the recovery. This will include the following items:

Antibacterial soap as a disinfectant
Laxatives or stool softeners as pain medicines often cause constipation
Loose and comfortable clothes
Entertainment items, including movies, books, magazines, etc.

The patient will also be advised to reduce the salt intake as it can increase the risk of fluid retention, resulting in swelling.

On the Night/Day of Surgery

On the day when the surgery is scheduled, the patient will be asked to follow the instructions below:

Take a bath using antibacterial soap to cleanse the body.
Avoid eating or drinking anything 6-8 hours before the procedure.
Do not wear any accessories or jewelry, as they will be removed before taking the patient to the operating room.
Bring the ID proof, insurance documents (if applicable), alternate pair of clothes, test reports, etc., to the hospital.

Once the patient is admitted to the hospital, the surgeon’s team will run some routine tests and ask the patient to sign the consent form before proceeding.

Breast Augmentation Surgery Procedure

Before the surgery, the patient is given a hospital gown to wear, and an IV line is inserted into her arm to administer medicines. The patient is then taken to the operating room and asked to lie on the operating table. The exact steps of the procedure will vary based on the incision technique used. Here is a general overview of the breast augmentation procedure using breast implants:

The surgeon will draw the incision lines and mark specific areas to guide the procedure. The markings are usually done around the breasts and across the chest area.
Then, general anesthesia is administered to put the patient to sleep and numb the body. This ensures that the patient doesn’t feel any kind of pain or discomfort during the procedure. In some cases where general anesthesia is not suitable, local anesthesia with IV sedation may be done to numb the patient.
The incision techniques that can be used for breast augmentation surgery are:
- Inframammary Incision: In this technique, the incision is made across the breast crease, also called the inframammary fold. It is the most commonly used technique as it allows the surgeon to place even large-sized implants with utmost precision.
- Periareolar Incision: In this technique, the incision is made around the perimeter of the areola in a crescent shape. This method is particularly helpful in minimizing the scarring as the incision is placed in the areola (dark-colored area around the nipple). However, it can also affect the woman’s ability to breastfeed by damaging the milk ducts present underneath.
- Axillary Incision: Also called transaxillary incision technique, it involves making an incision in the armpit and using an endoscope to guide the procedure. As the incision is made within the natural contour lines, the scarring is almost invisible. However, this method is not appropriate for use when silicone implants or small-sized silicone implants are used.
The upper skin layer is lifted to expose the underlying tissues and create a pocket to place the implant. The pocket can be formed directly below the breast glandular tissues, i.e., sub-glandular, or below the pectoral muscles, i.e., submuscular.
The surgeon will then insert the breast implant in the cavity or pocket and adjust the implant to ensure that a proper shape is achieved.
In some cases, a tissue expander may be used to stretch the pocket area and create enough room for the breast implant. It is commonly used when large-sized implants are being placed.
After the desired symmetry is achieved, the surgeon then closes the incisions using sutures, skin adhesives, or surgical tape. Then, the breasts will be covered with a sterile dressing, and a compression bra or garment will be placed over them for additional support.

After the procedure, the patient will be transferred to the observation room to monitor her vitals until she wakes up. After that, she will be transferred to the private/sharing/general ward. The doctor will ensure that the patient can walk and then discharge her within a few hours.

*Note: It should be noted that breast augmentation is only helpful in restoring breast volume, but it cannot address sagginess or droopiness. In such instances, a breast lift is often performed in conjunction with breast augmentation surgery.

What to Expect After Breast Augmentation?

Immediately after the surgery, the patient can expect to feel mild to moderate discomfort, nausea, confusion, and disorientation due to the lingering effects of anesthesia. These effects will subside in the next few hours. The patient won’t be able to see the results as the breasts will start to swell a short while after the surgery and will be covered with bandages.

Breast augmentation surgery is typically performed on an outpatient basis, which means that the patient can expect to go home on the same day until the doctor recommends otherwise. Before discharge, the doctor and his/her team will provide detailed post-op care instructions that will ensure that the recovery is smooth and the patient doesn’t experience any complications.

Risks and Complications Associated with Breast Augmentation Surgery

Typically, the plastic surgeon takes all the necessary measures to reduce the risks during and after the surgery. However, they can still arise and may require further intervention. Thus, it’s important to be aware of the risks and potential complications so that appropriate measures can be taken promptly.

Some of the risks and complications associated with breast augmentation surgery include, but are not limited to, the following:

Infection: Surgical incisions act as an entry point to the bacteria and microorganisms present over the skin. When they enter the body, the immune system tries to fight them, which results in symptoms like pain, itching, pus discharge, swelling, etc. Typically, the infection is managed through antibiotic medicines, but if they are ineffective, the patient may need further intervention.
Hematoma: This complication arises when blood starts to accumulate in the surgical site. Due to the pooling of blood, the area starts to swell and causes significant discomfort. Small hematomas get absorbed by the body, but large hematomas may require drainage.
Changes in Sensation: Surgical procedures often damage the nerves in the treated area, which can result in changes or loss of sensation in the breasts. This can cause numbness or tingling in the breasts that may go away after some time or persist even after the body has healed.
Reaction to Anesthesia: There are inherent risks of using anesthetic medicines, which can lead to respiratory distress, cardiac arrest, lidocaine toxicity, etc. If the patient experiences such reactions, the doctor will need to take immediate measures to mitigate them.
Capsular Contracture: This post-op complication arises when the scar tissues around the breast implant start contracting. It causes pain in the breast and often requires implant removal.
Breast Implant Displacement: In some cases, if the pocket created for the breast implant is larger than expected, the implant won’t be able to remain in the same position. It will move around within the cavity, which will affect the shape of the breasts. If the breast implant is displaced, the patient will need further treatment.
Rippling Effect: When the implant starts to settle in the breast pocket, there is a possibility that it’ll get wrinkled, and the breast will appear rippled, which will affect the overall shape and texture.
Implant Rupture: In some cases, the breast implant shell can rupture if there is a pre-existing defect, the scar tissues put too much pressure on the implant, or a tear develops in the shell during the surgery. If the saline implant ruptures, it deflates, and the solution leaks into the body and gets absorbed. This is not considered a medical emergency, and the patient will only need to get the implant removed and replaced, as desired. However, silicone implant rupture is considered a medical emergency as the silicone material doesn’t get absorbed by the body.

These complications can arise if the patient fails to comply with the pre and post-surgical care instructions. Though they are not life-threatening, they can compromise the results of breast augmentation surgery. Therefore, patients are advised to adhere to the care guidelines.

Recovery After Breast Augmentation

The recovery period is critical in determining the success of breast augmentation surgery. Typically, the recovery takes almost 4-6 weeks, and during this period, the patient needs to be very attentive and follow the care instructions strictly.

Take ample rest in the first few days after the procedure and arrange some time off from work.
Initially, the patient will feel discomfort or pain in the treated area, which will be managed with painkillers. Avoid driving while taking pain medications, as they can cause confusion and disorientation.
The breast region will feel tight and hard, and the patient may also experience breathing difficulties in the first 2-3 days.
Do not take a bath or soak the body in water, as it can increase the risk of infection.
Wear the compression garment 24×7 as long as advised by the doctor to support the breasts and reduce the swelling. Also, make sure that the garment is clean and dry.
Use a mild antibacterial soap, warm water, and a soft cloth to clean the surgical site. After cleaning, use soft wipes to dry the area and then apply the prescribed ointment or moisturizer to prevent dryness.
Eat a healthy and balanced diet rich in vitamins, antioxidants, and fiber. Also, reduce intake of salt, spicy foods, preserved foods, caffeine, and alcohol.
In the first week, sleep in an elevated position using pillows or a recliner. It will prevent fluid accumulation in the breasts, which can cause swelling.
Don’t engage in physical activities that can strain the breast area or tear the stitches. It’s okay to perform basic activities, such as walking, climbing stairs, sitting for work, etc., but with ample breaks and rest.
Do not let the treated area come in contact with direct sunlight, as it can cause hyperpigmentation and darkening of the skin.
If symptoms like persistent pain, redness, bruising, bleeding, swelling, deflation of the breasts, foul discharge, high fever, etc., appear, notify the doctor right away.
After the stitches are removed, the doctor will provide scar care tips to improve the appearance of the surgical scars.
Don’t drink alcohol or start smoking, as they affect the body’s healing abilities. Alcohol causes severe dehydration, and smoking affects the blood vessels, which impacts the fresh blood supply to the treated area.

The patient should understand that the swelling and bruising will take at least a month or even longer to subside. And during this period, it’s important to be patient and wait for the body to recover and allow the breast implants to settle. After the recovery is complete, the doctor will also advise the patient to start exercising to further improve their body contours.
