About Varicocele

When the veins in the scrotum enlarge, the condition is known as Varicocele. Varicocele affects sperm production and function that can potentially reduce fertility. Out of a hundred males, ten to fifteen have varicocele. Varicoceles are found to be more common on the left side of the scrotum as the male anatomy is not the same on both sides.

Treatment

If you consult a doctor for the diagnosis and treatment of your condition, the doctor will first feel for the non-tender mass above your testicle by physically examining your hydrocele. The doctor may also ask you to stand, take a deep breath to check for any abnormal enlargement of the veins in the scrotum. In order to confirm the diagnosis, the doctor may further recommend you to go through diagnostic tests like doppler/scrotal ultrasound and scrotal infrared digital thermography.

Let us now understand the various procedures of treating varicoceles:

  • Percutaneous embolization: This is a minimally-invasive daycare procedure that involves the injecting of a small catheter through the groin into the scrotal vein. Once the catheter is injected, a coil is inserted through the catheter into the varicocele to block the affected veins. This helps in redirecting the blood flow to the circulatory system via an alternate pathway through the healthy veins, resulting in the collapsing of the damaged veins.
  • Laparoscopic Varicocelectomy: A Laparoscopic varicocelectomy is performed under general anesthesia. During the procedure, the surgeon will make several small incisions around the abdomen and insert a thin, lighted scope (laparoscope). The abdomen will then be inflated using CO2 gas, which would allow the doctor to have a proper view of the internal structures. As soon as the swollen veins are identified, they will be cut and the ends will be closed off. Once the procedure is complete, all the tools will be removed and the incisions will be closed with sutures or clips.
  • Microscopic Varicocelectomy: This procedure is carried out with the help of a microscope. During this procedure, the surgeon makes a small incision above the scrotum and separates the testicular arteries and vas deferens in the scrotum. The surgeon then dissects further down to the spermatic cord, keeping the lymphatic drainage intact. Once the abnormal veins are encountered, each vein is meticulously dissected circumferentially and tied off to disrupt the flow, before draining the blood away from the testicle into the inner thigh and pelvis.
