  • What Is Circumcision?

Circumcision is the surgical removal of the foreskin covering the tip of the penis. There are numerous advantages of circumcision, like lower risk of diseases like urinary tract infections and sexually transmitted infections. In addition, it helps improve penile health and hygiene, as it becomes easier to clean and wash the penis.

The circumcision surgery procedure (Penis Skin Surgery) is relatively common for newborn baby boys in certain parts of the world. For some families, circumcision is a religious and cultural ritual; for some, it can be a matter of family tradition, preventive health care or personal hygiene. Circumcision after the newborn or neonatal period is also possible, but it is a more complex procedure.

What Is The Best Age For Circumcision Surgery?

Circumcision is a medical procedure that can be performed at any age when necessary. However, it is most often performed in children or teenagers. For children, infancy is the perfect time as, at this age, babies feel less pain, move less during the procedure, and will have no recollection of the operation. In addition, most religious traditions recommend early circumcision.

After the baby is older than 6 months and until around 3 years, it becomes a more challenging age for circumcision as the baby is more physically active and begins to understand something is happening without being able to understand that this is for his benefit.

The best age for adult circumcisions surgery is 14 years, as the patient would have gone through puberty. Patients of this age are cooperative and understand this is for their benefit.

Are you going through any of these symptoms?

What Are The Reasons For Performing Circumcision Surgery?

Circumcision surgery is a cultural or religious ritual for many Islamic and Jewish families and can also be a matter of personal hygiene or preventive health care. At times there is a medical requirement for circumcision to treat several medical conditions, including:

    • BalanitisBalanitis is a condition where the head of the penis gets inflamed. It is typically an infection of the skin on the head of the penis.
    • Balanoposthitis– Balanoposthitis is an inflammation that impacts both the glans penis and prepuce. It can appear at any age and has several causes, but poor hygiene and a tight foreskin can make it easier to get balanoposthitis.
    • Paraphimosis– Paraphimosis is a male genital problem in which the foreskin gets trapped behind the head of the penis and does not retract. If not treated in time, this condition can cut off the circulation to the tip of the penis and cause permanent damage.
    • Phimosis– Phimosis is the inability to retract the skin covering the head of the penis. It may appear as a tight ring of foreskin around the tip of the penis, preventing full retraction.

What Are The Health Benefits Of Circumcision Surgery?

The possible health benefits of circumcision surgery include:

    • Better hygiene. Circumcision makes it easier to wash the penis, which eventually leads to better hygiene and health.
    • Reduced risk of urinary tract infections. The risk of infections in males is low, but these infections are more expected in uncircumcised males. Severe urinary tract infections early in life can result in kidney problems later. Therefore, it is advised to go for circumcision to decrease the risks of urinary tract infections.
    • Reduced risk of sexually transmitted infections. Circumcised men might have less risk of certain sexually transmitted infections, like HIV. Yet, practicing safe sexual practices remains essential.
    • Prevention of penile problems. Occasionally, the foreskin on an uncircumcised penis can be difficult to retract which may lead to inflammation of the foreskin or the head of the penis. Hence, it is better to go for circumcision to prevent any penile complications.
    • Decreased risk of penile cancer. Although penile cancer is rare, it is less common in circumcised men. Also, cervical cancer is less common in the female partners of circumcised men.

The risks and complications of not being circumcised are rare and avoidable with proper care of the penis. However, circumcision might not be a good choice if certain blood-clotting disorders exist. Also, circumcision might not be suitable for premature babies who still need medical care in the hospital nursery or for babies with abnormalities of the penis.

    Risks Associated With Circumcision Surgery

Before undergoing any surgical procedure, knowing the possible risks and complications is essential. The most common risks and complications linked with circumcision are bleeding and infection. In addition, side effects related to anesthesia are also possible.

Rarely, circumcision might result in foreskin issues. For example:

    • The foreskin can be cut too short or too long
    • The foreskin might fail to heal adequately
    • The remaining foreskin might reattach to the end of the penis, needing minor surgical repair
    • Skin bridges
    • Inclusion cysts
    • Abnormal healing
    • Meatitis
    • Urinary retention
    • Necrosis of the Penis

    Types Of Circumcision Surgery

Over time and with advancements in medical technology, different types of circumcision procedures have evolved for adults and infants. The purpose is to make the process of circumcision easier and effective and far away from complications and pain.

    • Open Circumcision Surgery

Open surgery is a traditional procedure for circumcision. In this procedure, the surgeon creates incisions along the upper length of the foreskin and then removes them. It is a complicated procedure, and even a minor mistake by the surgeon during the procedure can result in serious complications.

Open surgery is associated with several postoperative complications, and the patients often experience a lot of pain and swelling in the penile region after the procedure. Also, since this circumcision procedure involves multiple incisions, cuts, and sutures, there is always an increased risk of infections.

It is a safe procedure of circumcision with high effectiveness that uses a circular stapler for circumcision. During the surgery, the stapler is fitted over the penis. Once fired, it removes the foreskin quickly and seals the incision using a silicone ring/non-absorbable staples.

The procedure of stapler circumcision takes around 30-45 minutes, and depending on the type of anesthesia, the patient can go home within 6-12 hours of the procedure. In addition, the recovery period is shorter than the open surgery technique. It will take you around 1 week to recover completely before you can return to your normal routine.

Laser circumcision is a state-of-the-art medical procedure that is the most latest circumcision technique available at the moment. Unlike stapler circumcision and open surgery, laser circumcision does not result in extensive incisions on the foreskin. Instead, in laser circumcision, the surgeon uses a high-intensity laser beam to remove the foreskin. The doctor also recommends it for balanitis, phimosis,  paraphimosis, posthitis, balanoposthitis, and other foreskin infection treatment.

There is no bleeding or pain involved in the procedure, and it only takes about 10-15 minutes for laser circumcision to complete. After the procedure, you can even go home on the same day. You won’t have to stay in the hospital, so there is no inconvenience.

Difference Between Partial And Full Circumcision Surgery

Full circumcision is a surgical procedure where the complete foreskin is removed. In a partial circumcision, only part of the foreskin is removed. In most cases, only the top portion of the foreskin is removed in partial circumcision, uncovering the tip of the penis but leaving the bottom of the penis head.

Partial circumcision is preferred instead of full circumcision for several reasons. You may simply want to keep some of the foreskins for aesthetic or personal reasons, or you may feel uneasy about removing the entire foreskin, especially if you have had it your whole life. In addition, partial circumcision may not always be a feasible option. If your foreskin has become stuck due to scar tissue formation, you may need to have the entire foreskin removed.

What Happens During The Circumcision Surgery Procedure?

Circumcision of newborn babies is done in the hospital nursery within 10 days after birth. For newborn circumcision, the baby will lie on his back with his arms and legs secured. The penis and the nearby areas will be cleansed, and an anesthetic will be injected into the base of the penis or applied to the penis as a cream. After that, either laser circumcision or stapler circumcision will be performed, and the foreskin will be removed. Next, the penis will be covered with an ointment, like an antibiotic or petroleum jelly, and wrapped with gauze. The entire procedure generally takes about 10- 15 minutes.

The circumcision procedure is similar for older boys and adults. However, the procedure might need to be performed under general anesthesia, recovery might take longer, and the risk of complications might be higher when done later in life.

What Can You Expect After The Circumcision Surgery Procedure?

It generally takes 7 to 10 days for the penis to heal. The tip of the penis can be sore at first, and the penis might look swollen, red, or bruised. You might also see some yellow fluid on the tip of the penis.

If your newborn is fussy as the anesthetic fades, hold him gently — however, avoid putting pressure on the penis. It’s OK to clean the penis as it heals. For newborn baby boys, change the bandage with each diaper change, and apply some petroleum jelly to the tip of the penis to prevent it from sticking to the diaper. Change the baby’s diaper often, and ensure the diaper is loosely fastened.

If there is a silicon ring instead of a bandage, it will fall away on its own — usually in a week. Once the penis heals completely, clean it with soap and water during regular bathing.

Major problems after circumcision are unusual. Contact the doctor immediately if:

    • Normal urination does not resume within 12 hours of the circumcision
    • There is continuous bleeding
    • There is foul-smelling drainage from the head of the penis

    FAQs About Circumcision Surgery

    Is circumcision surgery painful?

Like any surgical procedure, circumcision (Penis Skin Surgery) can cause pain. But using pain medications and anesthetics can reduce pain and discomfort. These can help both during the circumcision procedure and afterward.

Who should not have a circumcision?

Your healthcare provider may advise delaying circumcision or not doing it at all if the baby:

    • Has medical problems.
    • Has physical problems with the penis that may require surgery. (At times, surgeons will require the foreskin to correct the problem.)
    • Was born prematurely.

Does circumcision surgery affect sexual pleasure or fertility?

Circumcision (Penis Skin Surgery) does not affect fertility, and researchers believe it does not affect sexual pleasure.

Does circumcision impact erections? Can it cause erectile dysfunction?

Circumcision is a medical procedure to remove the foreskin, and the foreskin is not involved in producing erections. Practically, there is no risk that circumcision will cause erectile dysfunction or problems with erections if performed by a trained urologist. Many adult men who choose to be circumcised experience improved confidence after circumcision.

How soon after circumcision surgery can I have sex?

Depending on the type of circumcision, most men are advised to avoid sexual activity for at least 6 weeks, as any sexual activity before that can lead to complications with wound healing and raise the chances of contracting STIs.

Should newborns be circumcised using a stapler circumcision procedure?

Yes, newborns can be circumcised using a stapler circumcision procedure since it is completely safe for patients of all ages.

Do I need to visit a doctor to get the silicone ring used in stapler circumcision removed?

Usually, the silicone ring, along with the staples, will fall off on its own within 10-14 days once the incision has healed. However, if you experience severe pain, you can visit your doctor for a wound examination.

Which is better: partial or full circumcision surgery?

Several patients believe that partial circumcision:

    (i) helps in preventing infections and

(ii) helps avoid pain and sensitivity during sexual intercourse, as the external foreskin stays intact.

Furthermore, patients report that the full removal of the foreskin kills specific cells, which may lower sexual pleasure. Hence, it can be said that partial circumcision is better than full circumcision.

Can the foreskin grow back naturally after undergoing circumcision surgery?

Foreskin cannot simply grow back like a lizard’s tail. Instead, it takes 1-5 years of grueling stretching and a slew of strange devices.

Is circumcision operation covered under insurance?

In most cases, circumcision surgery is covered under health insurance as it is a symptomatic condition, but for patients who are undergoing circumcision due to aesthetic or religious reasons, the treatment may not be covered under insurance.

What is the cost of circumcision surgery in India?

The circumcision cost in India may range from Rs. 30,000 – Rs. 35,000. However, it is an approximate cost range that varies based on several factors, like the consultation of the surgeon or urologist, the experience of the surgeon, the location or city of the hospital where the circumcision takes place, the healthcare center, and more.

    What is Khatna?

Removing the foreskin of the penis is what circumcision or khatna is.

    Is circumcision good or bad?

The decision to circumcise is personal and can depend on various factors. Parents or individuals need to discuss the potential benefits and risks with a healthcare provider to make an informed choice. If circumcision is being considered for non-medical reasons, it’s also valuable to weigh cultural, religious, and ethical considerations.

    Does circumcision increase size?

Circumcision does not increase the size of the penis. If size concerns are significant, it’s important to discuss them with a healthcare provider for accurate information and guidance. Misconceptions about circumcision and size should not be a factor in deciding whether to undergo the procedure.
