What is ACDF surgery?

ACDF surgery stands for anterior cervical discectomy and fusion surgery. It is generally performed in patients with disc herniation or degeneration in the neck region. During the surgery, an incision is made over the throat region to remove the affected disc and replace it with a fusion graft to fuse the vertebral bones above and below the operated disc. 

Normally, the cervical spine is not stable enough to undergo laminectomy or discectomy alone, and fusion surgery is recommended for all patients to maintain cervical spine integrity. Depending on the complexity and type of surgery, severity of  condition, and other factors, there are three types of grafts commonly used – autograft bone, allograft bone, and bone graft substitute.

How to prepare for ACDF surgery?

What happens during ACDF surgery?

Before the surgery, a thorough diagnosis is necessary to ensure whether the surgery is necessary and to determine the type of surgery needed. The most common diagnostic tests performed before ACDF surgery are imaging tests like spine X-ray, MRI, CT scans, etc. 

Surgery is generally performed under general anesthesia, so you will have to stop eating anything after midnight on the day of the surgery. On arriving at the hospital, your attendant will perform your pre-surgery vital signs checkup and provide antibacterial soap and sterilized gowns so you can prepare yourself for the surgery.

The surgery generally lasts around 1-3 hours depending on the type and complexity of the operation. During the surgery, you will be laid on your back, and your neck area will be cleansed. If the surgery is being performed using a self-graft, then the hip bone is also prepped for the same.

The incision is usually around 2 inches long. Once the incision is made, the surgeon will use a fluoroscope to locate the affected vertebral discs. The damaged discs will be removed and bone spurs pressing down on the nerve roots will be removed (called foraminotomy) to make more space for nerves to exit the spinal canal. 

Finally, bone graft will be obtained, either from a cadaver or from the patient’s hip bone. The bone graft is placed on the operative region and fixed using a metal plate and screws. In some cases, an artificial disc may be used (disc replacement surgery), instead of graft for better spinal function, movement, and recovery.



What are the different types of graft used during ACDF surgery?

During ACDF surgery, a bone graft or similar substitute is introduced between the operated discs to prevent cervical spine collapse. The graft is usually fixed using a metal plate and screws and over the next 3-6 months joins the vertebrae to form a single bone. Different types of grafts are:

  • Autograft bone – taken from the patient’s hip bone. It provides a high success and fusion rate, but can be discouraged due to postoperative pain and discomfort.
  • Allograft bone – donor graft that is usually taken from a cadaver. This graft doesn’t have as many bone growing cells and proteins as a live donor graft, but can be recommended as it doesn’t cause the patient extra pain and discomfort.
  • Bone graft substitute – is an artificial substance made from plastic, ceramic or other biocompatible bioresorbable substances.

What are the benefits of ACDF surgery?

ACDF surgery is very effective in providing relief for the following conditions:

  • Bulging and herniated disc, i.e., the gel-like component of the spine bulges or ruptures through the surrounding disc wall
  • Degenerative disc disease, i.e., disc is degenerating to form bone spurs, leading to inflammation of the facet joints, drying out of the disc, loss of vertebral flexibility, etc.
  • Removal of bone spurs (osteophytes) due to arthritis
  • Symptoms caused due to cervical spine stenosis

What to expect after an ACDF surgery?

After the surgery, the patient will be moved to a recovery area for monitoring and observation. Once you are awake, your neck and spine movement will be assessed. Most patients who undergo either 1 or 2 level fusion surgery will be able to go home within 24 hours of the surgery. Most patients can drive within 1-2 weeks of the surgery.

The recovery period after ACDF surgery generally lasts around 4-6 weeks with proper postoperative care. Make sure you follow all the post surgery tips prescribed by your orthopedic surgeon, such as:

  • Follow a proper exercise and physical therapy routine, including proper lifting exercises
  • Maintain good posture while sitting, standing, moving, sleeping, etc.
  • Transform your work area to be more ergonomic
  • Maintain a healthy weight and BMI, etc.

 

What are the complications associated with ACDF surgery?

ACDF surgery is generally very safe, especially when it is performed arthroscopically. However, in some cases, there may be complications like:

  • Hemorrhage
  • Infection
  • Leakage of spinal fluid
  • Injury to soft tissues, blood vessels, or nerves around the operative region
  • Hoarseness and difficulty in swallowing due to laryngeal nerve damage
  • Fracture of metal plate
  • Extra stress on the surrounding discs

Which recovery tips should a patient follow after ACDF surgery?

While ACDF surgery is generally very effective in reducing spinal pain, it is not a permanent treatment, especially if the patient doesn’t follow proper postoperative care and instructions. Recovery tips a patient should follow to attain complete recovery without complications and to prevent recurrence are:

  • Don’t bend or twist your neck till recovery is complete.
  • Don’t lift anything heavier than 5-6 pounds and avoid all strenuous activities.
  • Don’t engage in sexual intercourse without approval from your doctor.
  • Avoid smoking and drinking alcohol as they can slow down your healing and react with your medications.
  • Make sure your incision stays dry. Cover it thoroughly while showering or bathing. Most patients are advised to start showering at least 2 days after the surgery. If there is infectious drainage, consult your surgeon immediately for the appropriate treatment.
  • Don’t apply any lotion or ointment on the incision site without your doctor’s approval.
  • Take your medications as prescribed. Do not skip any doses, take multiple doses at once or take any other medicines or supplements without approval.
  • Make sure you follow a healthy diet with plenty of water and fiber-rich foods following the surgery to avoid constipation.
  • Wear your brace as directed. Ice the surgical site for 15-20 minutes a day at least 3-4 times a day to prevent post surgical swelling.
  • Start walking around for at least 5-10 minute intervals as soon as possible. 

Types of neck surgery

  • Anterior Cervical Corpectomy – removal of cervical bone and discs, followed by fusion
  • Anterior Cervical Discectomy and Fusion – removal of herniated or degenerated disc to relieve pain and pressure on cervical nerves
  • Cervical Posterior Foraminotomy – shaving off of bone spurs to create more space for nerves exiting the spine
  • Cervical Laminectomy with fusion – removal of a section of bone from the rear part of the vertebrae to relieve nerve pain, followed by fusion
  • Micro Endoscopic Posterior Cervical Discectomy – arthroscopic minimally invasive cervical discectomy and fusion surgery
  • Cervical Artificial Disc Replacement – removal of damaged discs followed by insertion of a prosthetic disc to allow for better spinal flexibility and function
