What is Angioplasty & Stenting?

Angioplasty is a medical procedure used to treat a variety of conditions that affect blood vessels, including deep vein thrombosis (DVT). DVT occurs when a blood clot forms in a deep vein, typically in the legs. The clot can cause pain, swelling, and other complications if it blocks blood flow in the vein.

Angioplasty involves using a catheter to access the affected vein and inserting a small balloon into the vein. The balloon is then inflated to widen the vein and dislodge the clot, which is then pushed against the wall of the vein. This helps restore blood flow through the vein. Stenting involves the insertion of a small metal mesh tube called a stent into the affected vein to keep it open and prevent the clot from recurring. The stent acts as a scaffold, supporting the vein and keeping it from collapsing.

Both angioplasty and stenting are minimally invasive procedures and effective treatment options for DVT. However, these methods may not be a suitable treatment option for all patients.

Diagnostic Tests & Evaluations Done Before Angioplasty for DVT

Before recommending angioplasty and/or stenting for DVT treatment, the vascular surgeon will suggest several diagnostic tests and evaluations to assess the severity of the condition. This will help to confirm whether angioplasty is the appropriate treatment option for the patient.

Besides taking the medical history and doing a physical exam, the following tests are done:

  • Doppler ultrasound: This test uses sound waves to create images of the veins and measure blood flow. It can help identify the location and extent of the clot.
  • Venography: This is an X-ray test that involves injecting a contrast dye into a vein and taking X-rays to create images of the veins. It can help identify the location and extent of the clot.
  • Blood tests: Blood tests can help determine if you have an increased risk of developing blood clots or if there are other underlying medical conditions that may be contributing to DVT.
  • Pulmonary function tests: These tests can help determine if the clot has spread to the lungs and can cause pulmonary embolism.
  • Electrocardiogram (ECG): This test can help determine if the clot has affected the heart’s electrical activity.

What Happens During Angioplasty and Stenting for DVT?

During angioplasty and stenting for deep vein thrombosis treatment (DVT), the following steps are typically followed:

  • Before the procedure, the patient is given a local anesthetic to numb the area where the catheter will be inserted. The doctor may also administer a sedative to help the patient relax.
  • A small incision is made in the skin, and a catheter (a thin, flexible tube) is inserted through the incision and threaded through the veins to the site of the clot.
  • Once the catheter reaches the clot, a small balloon at the end of the catheter is inflated to compress the clot against the wall of the vein and restore blood flow. The balloon is then deflated and removed from the vein.
  • In some cases, a stent (a small, metal mesh tube) may be placed in the vein to keep it open and prevent the clot from recurring. The stent is inserted through the catheter and placed at the site of the clot. Once in place, the stent expands to keep the vein open. Multiple stents can be placed inside the vein to keep it open.  Once the angioplasty and stenting are complete, the catheter is removed, and the incision is closed with stitches or a small adhesive bandage.

After the procedure, the patient is monitored in a recovery area for several hours. As both procedures are minimally invasive, they are performed on an outpatient basis, and the patient is sent home after observation if everything is fine. The doctor may suggest an overnight stay in case the patient’s vitals are stable.

How to Prepare for Angioplasty and Stenting?

The preparation for angioplasty and stenting begins with the initial consultation with the doctor. The patient needs to inform the doctor about the prescribed and non-prescribed medications he/she is taking, existing medical conditions, and allergies (if any).

Other preparation tips will include the following points:

  • Avoid eating anything 6 to 8 hours before the procedure, and don’t take any non-prescribed medications.
  • Since sedatives are administered during the procedure, it’ll be best to ask a friend or family member to accompany you and drive back home.
  • Wear comfortable clothes that can be removed, or at least the area where the catheter will be inserted is easily accessible.
  • Stop using blood thinning medications at least 1-2 weeks before the procedure.
  • Quit smoking as it impairs nerve health and increases the risk of vascular problems.
  • Arrange for someone to help at home, as the patient will have limited mobility for a few days.

If there is any specific instruction that patients need to follow before angioplasty, the doctor’s team will inform them about the same.

What to Expect After Getting Angioplasty for DVT?

After getting angioplasty combined with stenting for deep vein thrombosis (DVT), the patient should expect the following:

  • Pain and discomfort in the site where the catheter was inserted.
  • Swelling and bruising in the treated area.
  • Limited mobility and activity restrictions for proper rest.

Before the patient is discharged, the doctor will provide detailed instructions for recovery and post-op wound care. The follow-up schedule is also given to the patient to monitor his/her progress. Several lifestyle changes are also recommended for the patient to reduce the risk of DVT in the future.

Recovery After Angioplasty

The recovery after angioplasty can vary depending on the extent of the procedure and your overall health. Most patients need around 2-4 weeks to recover completely. Typically, the patient can resume basic activities and get back to work within a week after the procedure. Throughout this period, it’s crucial for the patient to follow the instructions given by the doctor. Some general recovery tips are given below:

  • Follow all instructions from your doctor, including taking medications as prescribed.
  • Take the prescribed medications as directed by the doctor.
  • Keep the incision site clean and dry until it heals properly.
  • Rest and avoid strenuous activity for a few days after the procedure.
  • Avoid lifting heavy objects for a few days after the procedure.
  • Drink plenty of fluids to help flush the contrast dye out of your system.
  • Avoid hot tubs and swimming pools for a few days after the procedure.
  • Check the incision site daily for signs of infection, such as redness, swelling, or discharge.
  • Contact the doctor if any unusual symptoms, such as fever, chills, or chest pain, appear.
  • Attend all follow-up appointments with the doctor to monitor the progress and ensure that the blood vessels remain open.
  • Make lifestyle changes as recommended by the doctor, such as quitting smoking, eating a healthy diet, and getting regular exercise.

These tips will help the patient to reduce the risk of post-surgery complications and improve overall health.

Benefits of Angioplasty & Stenting for DVT

The benefits of angioplasty and stenting for deep vein thrombosis treatment (DVT) include:

  • Improved blood flow: Angioplasty and stenting can help restore normal blood flow to the affected area, which can help relieve pain, swelling, and other symptoms associated with DVT.
  • Reduced risk of complications: The treatment helps to reduce the risk of complications associated with DVT, such as pulmonary embolism.
  • Minimally invasive: Angioplasty and stenting are minimally invasive procedures, and hence are associated with fewer complications and a shorter recovery time compared to open surgery.
  • Less pain and discomfort: Minimally invasive treatment ensures that the patient experiences minimal to no pain and discomfort after the procedure.
  • Quick recovery: Most people are able to resume their normal activities within a few days after angioplasty and stenting, compared to several weeks or even months after open surgery.
  • Long-lasting results: The stents placed during angioplasty have been shown to be effective in treating DVT and keeping the affected veins open, thereby maintaining blood circulation.

Complications & Side Effects of Angioplasty & Stenting

Like all medical procedures, angioplasty and stenting for DVT can have potential complications and side effects. Here are some of the most common ones:

  • Bleeding or hematoma at the site of the catheter insertion.
  • Infection in the wound.
  • Allergic reaction to the contrast dye used during the procedure.
  • Kidney damage or impairment from the contrast dye used during the procedure.
  • Blood clots, which can sometimes form at the site of the stent placement or elsewhere in the body.
  • Restenosis, which is when the treated area becomes narrow again and blood flow is restricted.
  • Arterial dissection or perforation, which is when the catheter causes a tear or hole in the artery.
  • Stroke or heart attack, which are rare but serious complications that can occur during the procedure.
  • Anesthetic complications, such as allergic reactions or respiratory distress.

Most of these complications can be managed by the patient by following the doctor’s advice strictly.

FAQs Around Angioplasty

Can angioplasty be done without a stent?

Yes, angioplasty can be performed without a stent, but in some cases, a stent may be necessary to help keep the vessel open.

How long does angioplasty take?

The procedure usually takes between 30 minutes to a few hours, depending on the location and severity of the blockage. The duration of the procedure might be longer than expected if stenting is also done.

What is the alternative to angioplasty for DVT?

There are several alternatives available for angioplasty to treat DVT, including mechanical thrombectomy, IVC filters, thrombolysis, etc. The best method of treatment is suggested by the doctor after a detailed evaluation of the condition.

Are angioplasty and stenting covered by health insurance in India?

Most health insurance plans cover the cost of angioplasty and stenting. DVT is a serious condition whose treatment is deemed medically necessary. Thus, irrespective of the treatment procedure for DVT recommended by the doctor, the health insurance company will cover the expenses of the treatment.
