What is Subacute Appendicitis?

An appendix is a vestigial organ located at the junction of the small and large intestines in the digestive system. Inflammation of the appendix causes appendicitis. As the inflammation worsens, appendicitis pain typically increases and eventually becomes severe. Appendicitis is a medical emergency that almost always requires surgery as soon as possible to remove the appendix. If left untreated, the appendix might burst and spill harmful material and infectious fluids into the abdominal cavity.

There are three main types of appendicitis: acute appendicitis, subacute appendicitis, and chronic (recurrent) appendicitis. Subacute appendicitis is a term used to describe a condition where the inflammation of the appendix is not severe or acute. It falls somewhere between acute and chronic appendicitis in terms of pain progression. In subacute appendicitis, the inflammation of the appendix may develop gradually over a period of time, leading to a milder set of symptoms.

The signs and symptoms of subacute appendicitis are similar to those of acute appendicitis but are generally less severe. These symptoms may include abdominal pain, tenderness in the lower right abdomen, loss of appetite, mild fever, and occasional nausea or vomiting.

Diagnosis And Treatment of Subacute Appendicitis

Diagnosis

Diagnosing subacute appendicitis involves a combination of medical history assessment, physical examination, and diagnostic tests. Some common diagnostic tests for subacute appendicitis include the following:

  1. Medical History and Physical Examination: The doctor will ask about your symptoms and medical history and perform a physical examination. They will check for tenderness, inflammation, or pain in the lower right abdomen, which is a sign of appendicitis. However, it’s crucial to note that physical examination alone may not provide a final diagnosis.
  2. Blood Tests: Blood tests may be performed to assess the levels of infection markers, like white blood cell count and C-reactive protein (CRP). High levels of these markers may suggest the presence of appendicitis.
  3. Imaging Tests: Your doctor may also conduct certain imaging tests, which include the following:

  • Ultrasound: Ultrasound imaging uses sound waves to produce pictures of the internal organs. It can help visualize the appendix and notice signs of inflammation or obstruction.
  • Computed Tomography (CT) Scan: A CT scan provides clear and detailed cross-sectional images of the abdomen. It can help identify the inflamed appendix and determine the severity of the condition.
  • Magnetic Resonance Imaging (MRI): MRI may sometimes be used to evaluate the appendix and surrounding structures. It can be helpful when other imaging modalities are inconclusive.

The specific diagnostic approach may differ depending on the individual case. The doctor will consider various factors, including the severity of symptoms, medical history, etc., to decide which tests are most appropriate in confirming the diagnosis of subacute appendicitis.

Treatment

The treatment for subacute appendicitis involves surgical removal of the appendix (appendectomy) to prevent further complications. The surgical procedure for subacute appendicitis is similar to that of acute appendicitis. It can be performed using different techniques, including open or laparoscopic surgery.

  • Open surgery- Open surgery, also known as open appendectomy, is a surgical procedure to remove the subacute appendix through a larger incision in the lower right abdomen.

During this procedure, the surgeon will make a single incision in the lower right abdomen, typically about 2 to 5 inches long to have direct access to the appendix. Once the appendix is accessed, the surgeon will isolate and detach it from its attachments to the surrounding tissues. The blood supply to the appendix will be ligated to prevent bleeding. The appendix is then carefully removed from the abdomen. After removing the appendix, the surgeon will close the incision using sutures or staples.

  • Laparoscopic surgery- Laparoscopic surgery is a minimally invasive surgical option to treat subacute appendicitis. It offers several advantages over traditional open surgery, including smaller incisions, reduced postoperative pain, shorter hospital stays, faster recovery, and minimal scarring.

During this surgery, the surgeon makes several small incisions (typically 4 to 6) in the abdomen, including one near the belly button. A laparoscope, a thin tube with a camera and light source, is then inserted through one of the incisions to provide a clear view of the abdominal cavity. The surgeon will now carefully identify and dissect the appendix from its surrounding tissues. After removing the appendix, the incisions are closed with sutures, surgical glue, or adhesive strips.

Are you going through any of these symptoms?

What Is The Most Effective Treatment Option For Subacute Appendicitis?

Laparoscopic surgery is the safest and most effective surgical option for subacute appendicitis. It is often considered the most effective treatment option due to its advantages over open surgery. Here are some reasons why laparoscopic surgery is preferred for subacute appendicitis:

  • Minimally Invasive: Laparoscopic surgery is a minimally invasive procedure that results in less trauma to the tissues and results in smaller scars compared to open surgery.
  • Faster Recovery: Laparoscopic surgery has a quicker recovery time than open surgery. Patients often experience less post-operative pain, have shorter hospital stays, and can return to everyday activities sooner.
  • Reduced Complications: Laparoscopic surgery has been associated with lower rates of surgical site infections and wound complications compared to open surgery.
  • Improved Cosmetic Outcome: The smaller incisions in laparoscopic surgery cause less prominent scars, benefiting patients concerned about cosmetic appearance.
  • Less Blood Loss: Laparoscopic surgery generally involves less blood loss than open surgery.
  • Lower Risk of Adhesions: Adhesions, which are bands of scar tissue that can develop after surgery, are less common with laparoscopic surgery. This can minimize the risk of complications related to adhesion formation, such as bowel obstructions.

How To Prepare For An Appendectomy?

Before your appendectomy, your doctor will likely explain the procedure in detail and answer any questions you have. It’s essential to be honest with your doctor about any medications you are taking, including over-the-counter medications and supplements, as some of these may need to be discontinued before the procedure.

It is necessary to avoid eating or drinking anything for at least eight hours before the procedure. It is also important to report to the doctor if you are pregnant or are suspected of being pregnant, if you are allergic or have a sensitivity to latex or some anesthesia, and if you have bleeding disorders.

Since anesthesia will be used during the surgery, it is important to arrange for someone to drive you home after the procedure. You may not be in a suitable condition to drive yourself due to the effects of the anesthesia.

What Are The Post-Operative Care Tips To Follow After An Appendectomy for Subacute Appendicitis?

After undergoing an appendectomy for subacute appendicitis, it is essential to follow proper post-operative care instructions to enhance healing and minimize complications. Here are some tips for post-operative care:

  1. Incision Care: Keep the incision area clean and dry. Follow the instructions provided by your surgeon for wound care, such as changing dressings, cleaning the incision site, and applying any suggested ointments or creams.
  2. Pain Management: Take pain medications as prescribed to address post-operative pain or discomfort. If the pain persists or worsens, contact your surgeon instantly for guidance.
  3. Physical Activity and Rest: Follow your surgeon’s advice regarding physical activity and rest. Initially, you may need to limit strenuous activities and avoid heavy lifting. After a few weeks, you can gradually increase your activity level as your surgeon advises.
  4. Proper Nutrition: Maintain a healthy and balanced diet to enhance healing. Include plenty of fruits, vegetables, lean proteins, and whole grains in your diet. Stay hydrated by drinking adequate fluids unless otherwise instructed by your surgeon.
  5. Avoid Straining: To prevent strain on the surgical site, avoid activities that may strain the abdominal muscles, such as heavy lifting or intense physical exertion. It’s essential to give your body time to heal properly.
  6. Follow-up Appointments: Attend all scheduled follow-up appointments with your surgeon. These visits are vital for monitoring your recovery progress and managing any concerns or complications that may arise.
  7. Watch for Signs of Complications: Be aware of potential signs of complications, like increasing pain, redness, inflammation, discharge, or fever. If you experience any unusual symptoms, contact your surgeon instantly.

What Happens If Subacute Appendicitis Is Left Untreated?

If subacute appendicitis is left untreated, it can be risky and potentially lead to complications. The complications that can occur include the following:

  1. Progression to Acute Appendicitis: If left untreated, subacute appendicitis can worsen and evolve into acute appendicitis, which is a medical emergency. Acute appendicitis is associated with extreme abdominal pain, increased risk of appendix rupture, and potentially life-threatening complications.
  2. High Risk of Appendix Rupture: As subacute appendicitis advances, the appendix may become more swollen. This increases the risk of the appendix rupturing. A ruptured appendix can lead to the spread of bacteria and infection throughout the abdominal cavity, causing peritonitis. Peritonitis is a severe and life-threatening complication that needs immediate medical attention.
  3. Complications and Prolonged Recovery: Delaying treatment can increase the likelihood of complications associated with appendicitis, such as abscess formation, bowel obstruction, or the development of adhesions (scar tissue). These complications can lead to a more complex surgical procedure, longer hospital stays, and prolonged recovery periods.
  4. Ischemia and necrosis. Severe swelling in your appendix can cut off the blood supply (ischemia,) which can increase inflammation and eventually cause the tissues to decay (necrosis). Ischemia and necrosis of the appendix are severe complications because they can increase the risk of appendix rupture. When the tissues of the appendix are not obtaining sufficient blood supply, they become more prone to bacterial infection and tissue breakdown.
  5. Phlegmon. At first, the infection might not spread far beyond the appendix. It might form an abscess, a pocket of pus, on the outside of the appendix. However, the appendix can also develop a mass around itself called a phlegmon, which can burst. Phlegmons are considered a major complication of appendicitis because they can cause persistent pain, fever, and discomfort. If left untreated, the phlegmon can progress, potentially leading to other complications like appendix rupture.

What Are The Possible Complications Of Appendectomy?

Here are some possible complications of appendectomy:

  • Infection: Infections can occur at the incision site or within the abdominal cavity after an appendectomy. This can result in wound infections and abscess formation.
  • Bleeding: There is a risk of bleeding during or after the surgery. In most circumstances, bleeding can be controlled during the procedure. However, in rare instances, additional surgical intervention may be required.
  • Adverse Reaction to Anesthesia: Some people may have an adverse reaction to anesthesia, which can lead to complications such as respiratory issues, allergic reactions, or cardiovascular problems.
  • Hernia: In some instances, a hernia may form at the site of the incision, resulting in a bulge or protrusion of tissue. This may need additional surgical intervention to repair the hernia.
  • Persistent Symptoms: In rare cases, individuals may continue to experience abdominal pain or other symptoms after an appendectomy. This may be due to underlying conditions or complications that need further evaluation and treatment.
  • Injury to Nearby Organs: During the surgery, nearby structures like the intestines, blood vessels, or bladder may be accidentally injured. Although this is uncommon, it may need additional surgical repair.

Frequently Asked Questions

What is the size of subacute appendicitis?

The size of subacute appendicitis can range from a few millimeters to several centimeters in diameter. However, the size alone may not always be a dependable indicator of the severity or stage of appendicitis.

Is subacute appendicitis serious?

While any appendicitis should be taken seriously, subacute appendicitis can have varying degrees of severity. The severity can depend on factors like the extent of inflammation, the existence of complications, and the person’s overall health. Without appropriate medical intervention, subacute appendicitis can progress to acute appendicitis, which is considered a medical emergency. It is essential to seek medical attention if you suspect you have appendicitis, regardless of whether it is acute or subacute.

Can stress cause subacute appendicitis?

While stress itself is not considered a direct cause of subacute appendicitis, it is possible that stress can indirectly lead to the development or worsening of the condition. Stress can impact the body in several ways, including its impacting the immune system and digestive processes. Prolonged or chronic stress can weaken the immune system, potentially making a person more susceptible to infections that could cause appendicitis. Also, stress may affect bowel movements, contributing to changes in stool consistency and potentially increasing the risk of blockage within the appendix.

Is appendix operation painful?

The primary goal of an appendix operation is to remove the inflamed appendix. The procedure is typically performed under anesthesia, meaning you will be unconscious and not experience any pain or discomfort during the operation. However, after the operation, it is common to experience some pain and discomfort at the incision site or in the abdominal region. The level of pain experienced after an appendix surgery can vary from person to person.

What is the recovery time after an appendectomy for subacute appendicitis?

The recovery time after an appendectomy for subacute appendicitis varies from person to person but is generally relatively short. Most individuals can return to regular activities within one to two weeks after the surgery. However, strenuous activities such as heavy lifting, running, swimming fast, etc., should be avoided for a few weeks to allow the surgical site to heal properly.

How long does it take for subacute appendicitis to progress to acute appendicitis?

The time it takes for subacute appendicitis to progress to acute appendicitis varies from person to person. In some circumstances, it may progress rapidly within a day or two, while in others, it may take several days or weeks. It is difficult to predict the exact timeline, so it is vital to seek medical attention promptly when symptoms occur.

What foods to avoid if you have subacute appendicitis?

Some foods to avoid if you have subacute appendicitis include spicy and greasy foods, carbonated and caffeinated beverages, and dairy products. It is because these foods can potentially aggravate discomfort associated with subacute appendicitis. However, it is important to consult with a healthcare professional for personalized advice regarding your specific condition and dietary needs.

What is the cost of an appendectomy in India?

The cost of an appendectomy in India may range from Rs. 40,000 to Rs. 70,000. However, the cost of the surgery is not specific and depends on distinct factors like the location of the hospital where the surgery takes place, the healthcare center, the fees of the surgeon, the severity of the condition, and more.
