  • Why Is Circumcision Carried Out In Babies?

Circumcision is the surgical process of removing the foreskin. The foreskin is the retractable fold covering the end of the penis. During circumcision in babies, a surgeon removes a portion of the foreskin and reattaches the remaining section to create a shorter section of skin.

You are not legally or medically required to circumcise your baby boy. But, some families decide to circumcise their baby based on cultural or religious beliefs. Others prefer to circumcise for hygiene or appearance. While some others choose not to circumcise their babies.

Premature babies or those with particular medical concerns may require to wait longer until they can be circumcised. Some babies are born with biological problems affecting the penis that require surgery. However, they may not be circumcised because doctors use the foreskin as part of the reconstructive operation. Sometimes, families decide to wait to circumcise their baby boy so that their child can choose later. Circumcision after the newborn period is usually done in an operating room, with the child getting general anesthesia, which comes with additional risks.

Types Of Circumcision Procedure In Babies

There are different techniques for performing circumcision surgery in infants. However, the choice of circumcision technique depends on the doctor’s preference and experience and the parents. The common methods of circumcision in baby boys are laser circumcision, stapler circumcision, and open circumcision.

    Laser Circumcision

Laser circumcision is one of the safest and most advanced circumcision procedures in babies. During the laser circumcision procedure, the pediatrician or obstetrician removes the foreskin using a high-energy laser beam. Since a laser is used to make an incision, there is little to no penile bleeding, and the wound edges are clean.

Laser circumcision takes only 10-15 minutes and has a shorter recovery time. It is also preferred as a foreskin removal surgery for baby boys as it has very little postoperative pain and discomfort.

    Stapler Circumcision

Most decisions about circumcision in babies are based on cultural, religious, or personal reasons. Though circumcision in baby boys is not a medical necessity, the benefits of circumcision in babies outweigh the risks of indicative medical conditions.

Stapler circumcision is a specialized circumcision technology in infants that utilizes a surgical stapler to cut and seal the cut skin edges instead of traditional sutures. The circumcision stapler is a single-use device, which is a sterile, hand-held, manual surgical instrument preloaded with non-bioabsorbable skin staples intended to be used to perform a circumcision in babies. It is designed to cut the foreskin and apply a circular row of staples proximal to the cut line. After the stapler circumcision is completed, the baby gets discharged on the same day, within a few hours, so that he can rest and recover at home.

    Open Circumcision

Open circumcision is a traditional procedure for circumcision in babies where the surgeon creates incisions along the upper length of the foreskin and then removes them. Open circumcision in babies is associated with certain postoperative complications, but it also has the advantage that any injury caused can be identified instantly and addressed intraoperatively.

Are you going through any of these symptoms?

What Is The Circumcision Procedure For Babies?

Most circumcisions, whether laser or stapler, are performed within 24–72 hours after birth by a pediatrician or obstetrician. You can also take your little one to the pediatrician’s place within the first 10 days after birth to have the circumcision performed there. It’s considered safest to have a circumcision performed in a medical setting.

The procedure for circumcision, whether it is laser circumcision or stapler circumcision, usually takes between 15 to 20 minutes. You can stay with your baby during the entire procedure. Usually, the baby will be taken to a procedure room and placed on a table with soft restraints for their legs and arms..

Furthermore, an anesthetic will be injected at the base of the penis with a needle or applied as a cream to the skin. Then, depending on your physician’s preferred method, a special clamp would be placed around the foreskin to cut off the blood supply and remove the foreskin.

What Are The Benefits Of Circumcision In Baby Boys?

Babies who are circumcised have a lower likelihood of:

    • getting HIV and other sexually transmitted diseases when they are older
    • acquiring urinary tract infections (UTIs), particularly in the first year of life
    • getting penile cancer. However, getting penile cancer is rare in both circumcised and uncircumcised infants.
    • developing inflammation, irritation, or infection of the penis because it is more comfortable to keep a circumcised penis clean.

What Are The Risks Of Circumcision In Babies?

Like any surgical procedure, circumcision in babies has some risks. These are rare but include the following:

    • Bleeding, which is usually a small amount. Be sure to inform the doctor about any bleeding conditions in the family, as these can cause more bleeding after circumcision is completed in infants.
    • Infection, which is mild and easy to cure
    • More skin or less skin removed than planned by the parent and the doctor
    • Scar tissue formation

    Circumcision Side Effects On Babies

Besides the initial swelling, infection and bleeding are the two  most common side effects of circumcision in babies. Other possible complications of circumcision in babies can include the following:

    • tenderness around the scar
    • the need to remove stitches that haven’t dissolved
    • sometimes, another operation is required to remove some more foreskin from around the head of the penis
    • inadequate skin removal
    • side effects from the method or medicine used for pain relief.

Recovery Tips After Circumcision In Babies

The parent should follow the given tips to enhance the healing process of their baby after circumcision surgery:

    1. The penis will take around 8 to 10 days to heal. The area may be red or swollen for a few days, and you may notice some yellow discharge, which should reduce as it heals. Talk to your baby’s healthcare provider about what to expect after the circumcision.
    2. Follow the instructions given by the doctor who did the circumcision about caring for the dressing, using petroleum jelly, keeping the area clean, and bathing.
    3. Call your health care provider immediately if:
      • You see continuous bleeding at any time during the healing process.
      • The swelling and redness around the circumcision do not start to go down in 48 hours.
      • Your baby develops a fever.
      • Your baby seems to be unwell.
      • Your baby does not pass urine within 12 hours of the procedure.
      • There is a greenish or foul-smelling discharge from the penis.

Common FAQs Around Baby Circumcision Surgery

Who performs circumcision in baby boys?

The circumcision procedure may be done by a pediatrician or a general surgeon. In some cases, circumcision in babies may be done in a non-medical setting for cultural or religious reasons. If this is the case, the person doing the circumcision should be well-trained in how to do the procedure, relieve pain, and prevent infection.

Do babies feel pain during circumcision?

Yes, newborn babies feel pain. Therefore, the practitioner performing the circumcision should use a local anesthetic given by a needle in the area where the circumcision is done. Additional methods of relieving pain include topical anesthetic cream and acetaminophen.

When should circumcision not be done?

Circumcision should not be performed if:

    • The baby is born very early
    • The baby has specific issues with his blood or a family history of bleeding disorders
    • The baby has some congenital abnormalities

How to keep the circumcised area clean?

Some of the ways to keep the circumcised area clean are:

    1. Use mild soap to wash the penis gently.
    2. Remove any stool with water during diaper changes.
    3. Change diapers frequently so that urine and stool do not cause infection. Signs of infection include persistent redness, swelling, or fluid that looks cloudy and forms a crust. Call your healthcare professional immediately if you notice any of these signs.

What should I consider when deciding to go for circumcision for my baby?

It is essential to have all the information about the possible benefits and risks of the circumcision procedure before making a decision. You may consider future health benefits for the baby, religious or cultural beliefs, and personal preferences or social concerns.

What is used to prevent pain for the baby during the circumcision procedure?

While historically, circumcision procedures were done without pain medicines, that is not the case in modern medicine. In most circumstances, a combination of local anesthetics is used to numb the circumcision site and prevent pain during and after the procedure.

What is the best time for a baby to be circumcised?

In most cases, a baby boy is circumcised as an infant. Infant circumcision is usually done anytime in the first four months of a baby’s life. Most medical experts accept that the earlier the circumcision is done on a baby, the easier it is. This is because the baby will have a smaller penis at a younger age. Also, the younger they are, the less likely they are to be aware of their surroundings.
