  • What Is Cystolithotomy?

Cystolithotomy is a traditional, open procedure for painful bladder stones. This procedure involves the removal of a bladder stone via a cut (approximately 10cm) in the lower abdomen. Most bladder stones can be dealt with by an endoscopic (telescope) procedure to avoid the need for a surgical incision. If stones are very large, it is often best to perform an open operation. These options will be discussed with you before surgery.

Stones form in the bladder for a variety of reasons. In men, the most common cause is bladder outflow obstruction from an enlarged prostate.

    Diagnosis

A thorough diagnostic procedure before cystolithotomy is as crucial as the treatment procedure itself. The cystolithotomy diagnostic procedure consists of some standard tests that help doctors perform this surgery with much more precision. Some common tests performed before cystolithotomy include:

A physical exam: A physical exam before cystolithotomy comprises a thorough assessment of the lower abdomen. The doctor looks for enlargement in the bladder and may even perform a rectal exam to rule out enlarged prostate. Additionally, the doctor asks questions about the symptoms you’re experiencing, your medical history, lifestyle habits, and so on.

A urine test: In the urine test, the doctor will collect a sample of your urine to examine it under the microscope. Doctors look for microscopic amounts of blood, bacteria, and crystallized minerals in the sample and even rule out other factors, such as urinary tract infections, that may be causing the stones.

Ultrasound: An ultrasound before cystolithotomy helps doctors identify bladder stones’ size, number, and location. Sound waves are bounced off organs and other structures of the body to produce images.

X-ray: The doctor might perform an X-ray of the kidney, ureter, and bladder to figure out if you have bladder stones. However, this test can not be used in the case of all bladder stones.

CT Scan: This test uses X-rays and computers to provide clear images of the body’s internal structures. The computer quickly scans the body and can even detect very small stones. This test is considered the most effective for identifying all bladder stones.

    Preparation

Here’s how you can prepare for your cystolithotomy surgery:

    1. This surgery is performed under general anesthesia. Therefore, if you’re allergic to it, make sure you discuss it with your doctor beforehand.
    2. If you’re on medication, especially blood thinners, inform your doctor before the surgery. Some medicines can affect the procedure’s efficacy and may cause other complications as well.
    3. If you have questions about the surgery, ensure you address them before the surgery.
    4. Do not eat or drink anything for at least 6 hours before the surgery to avoid anesthesia-related complications.
    5. Remember to bring an attendant along with you.
    6. Leave all precious items at home.

    Procedure

Here’s how the cystolithotomy procedure is performed:

    • The concerned patient received general anesthesia and remains sedated throughout the procedure.
    • The doctor then performs a cystoscopy to examine the bladder and figure out the location and number of stones.
    • The surgeon makes an incision in the lower abdomen in order to access the bladder.
    • Then, a foley catheter is inserted through the incision to reach the bladder, after which sterile water is passed into the bladder.
    • The skin is incised over the ventral midline pelvis, right between the rectus muscles. After this, an extraperitoneal approach is used to make a dissection down in the anterior bladder wall. Then, the detrusor muscles are incised vertically.
    • The bladder is opened big enough to extract the stones. In case the stone is too large and requires a large incision, then a suprapubic catheter is inserted and left in place.
    • Finally, the bladder is sealed with absorbable stitches.
    • In some cases, the doctors leave an extraperitoneal drain near the bladder closure.

    After Procedure

The patients might have to spend a day or two at the hospital after the open cystolithotomy procedure. The doctor will examine the patient’s overall health thoroughly before proceeding with discharge. The catheter will be left for 7 to 14 days. Complete recovery after cystolithotomy may take up to several weeks. Till then, the patient is advised to take complete rest at home. Here are some ways in which you can boost your recovery after cystolithotomy:

    • Do not overexert yourself by indulging in strenuous activities.
    • Avoid weight lifting and heavy workouts.
    • Maintain a healthy diet.
    • Do not miss your follow-up sessions with the doctor.

    Risks of Cystolithotomy

All procedures accompany the risk of potential complications and side effects. While this procedure is quite safe and rarely causes any issues, anyone undergoing it must be aware of the associated risks. Some common risks associated with this procedu

    Frequently Asked Questions on Cystolithotomy

    Is a cystolithotomy procedure painful?

No. All patients receive general anesthesia before the procedure and, therefore, remain completely sedated during the procedure. Some patients might feel minor discomfort after the procedure, which is generally manageable through medicines.

When will I be able to go home after Cystolithotomy?

Most patients have to spend a couple of days in the hospital after cystolithotomy surgery. However, the overall duration of the hospital stay can vary from patient to patient. The doctors analyze the patient’s health thoroughly before preparing them for discharge.

What are some other treatments for bladder stones?

Aside from Cystolithotomy, which is an open surgical procedure to treat bladder stones, some other surgical methods are also widely used. Cystolitholapaxy is another procedure for bladder stone treatment that involves breaking the stones into smaller fragments using a laser or ultrasound device and vacuuming or washing them away.

    Is Cystolithotomy covered under insurance?

Yes. Custolithotomy is performed in case of large or too many bladder stones that fail to respond to endoscopic treatment. Therefore, this surgery is considered a medical necessity and is covered under insurance. The insurance coverage can vary, and therefore, it is advisable to get in touch with your insurance provider to get clarity on it.

Is Cystolithotomy always required for bladder stone removal?

Not necessarily. Cystolithotomy becomes necessary when bladder stones are too large or too many in number and can’t be treated through other treatment methods. In other cases, less invasive treatment methods such as Cystolitholapaxy may be enough.

What are some symptoms of bladder stones?

Bladder stones can cause some discomforting symptoms, including:

    • Pain in the lower abdomen, which can vary from mild to severe. Some patients may also experience pain in or around the penis.
    • Increased frequency of urination
    • Pain or difficulty while urinating
    • Dark-coloured of, cloudy urine
    • Blood in the urine, etc.

Can bladder stones be treated with home remedies?

Typically, home remedies do not offer much relief in the case of bladder stones, and surgical removal is almost always necessary. However, in some cases, drinking enough water can help small stones pass naturally through the system.
