What is an epidermal inclusion cyst?

An epidermal inclusion cyst is a benign cyst usually found on the skin. It is one of the most common cutaneous cysts. These cysts can occur anywhere on the body, typically present as nodules directly underneath the patient’s skin. They can range in size from a few millimeters to several centimeters. They are generally asymptomatic. But, as the keratin accumulates within the cyst, the cyst may increase in size, become sore or painful, and eventually rupture, leading to drainage of white or yellow cheese-like material. The chances of epidermal inclusion cysts are higher in men than in women. Also, the chances of developing epidermal inclusion cysts are high in the 20s to 40s. Epidermal inclusion cysts are also known as infundibular cysts, keratin cysts, and epidermoid cysts.

In this procedure, a fenestrated drape is placed on the patient, with the lesions beneath the fenestration. After that, a blade is used to create a stab incision into the center of the cyst. A small tipped hemostat is placed into the cyst, the tip gently opened, and compression applied to allow the cyst contents to pass through the opening. This procedure is performed under the influence of general anesthesia.

What causes Epidermal Inclusion Cysts?

Most epidermal inclusion cysts occur when the keratin protein is trapped below the skin because of disruption to the skin or to a hair follicle. Some genetic conditions like Gardner syndrome (familial colorectal polyposis), Gorlin syndrome (basal cell nevus syndrome), and favre-racouchot syndrome are also associated with the development of multiple epidermal inclusion cysts. 

Some medications can also increase your risk of developing epidermal inclusion cysts, such as: 

  • Cyclosporine
  • Imiquimod
  • BRAF inhibitors

Epidermoid cysts are mostly small, non-cancerous lumps that develop under the skin. They are typically painless and do not lead to complications. An epidermal inclusion cyst may show malignancy if it has any of the following characteristics: 

  • Redness, swelling, and drainage of the cyst
  • A fast rate of growth
  • Cyst having a diameter larger than five centimeters
