    The recovery time for anal fistula surgery depends on the type of surgery a person has, their overall health, whether they have an infection, and the surgeon’s skill. Success rates also vary and can affect the recovery period. 

    Your wound after your anal fistula surgery should heal within six weeks. However, depending on the size of the wound, you may need to return to the hospital daily to change your dressing. Your doctor will let you know how long you will need dressings. 

    You’ll likely experience some discomfort for a week after anal fistula surgery. You can take over-the-counter painkillers like paracetamol or ibuprofen for pain relief. Also, constipation can be common after surgery. Your doctor may suggest a laxative to soften your poop and help reduce discomfort. 

    After a person fully recovers, the fistula heals in most cases, and the person will not have lingering issues like incontinence. But if there are surgical complications, a person may require additional treatment or surgery.

    Post-Operative Care After Anal Fistula Surgery

    Here are some tips to help with recovery from anal fistula surgery:

    • Avoid sexual intercourse or inserting anything into the vagina or rectum until a doctor says it is safe.
    • Follow up with your doctor after the surgery. Most doctors schedule a visit a few weeks after surgery to ensure recovery is going well.
    • Drink plenty of water to lower the risk of constipation and keep stools soft.
    • Avoid constipation by consuming high-fiber foods or taking a stool softener.
    • Rest when you feel tired. Getting enough sleep will help you recover smoothly. 
    • Be active. Walking is a good way to speed up your recovery.
    • Ask your doctor when you can drive again.
    • Shower or take baths as usual. Pat the anal area dry with a towel when you are done bathing.
    • After a bowel movement, use a wipe or take a shower or sitz bath to cleanse the anal area gently.
    • You may have bandages over the opening of the fistula. Your doctor will tell you how to take care of the bandages. Therefore, make sure you follow the instructions diligently. 
    • Apply ice for the first 5-8 days after your surgery. While the post-op dressing is in place, the application of ice should be continuous. Once the dressing comes out, ice should be applied for 15 to 20 minutes, 3-4 times daily. Care must be taken with ice to avoid frostbite.
    • Place a maxi pad or gauze in your underwear to absorb drainage from your fistula while it recovers.
    • Do not use soap or put salt or scented products in the water until the wound has healed completely. These could irritate the wound. 
    • Sit in a few inches of warm water (sitz bath) for 10 to 15 minutes. Then pat the area dry. Do this as long as you have pain in your anal region.
    • Your doctor may ask you to digitate your wound. Move a finger along your wound to prevent the topmost layer of skin from healing too quickly. This might be uncomfortable, but it can help stop your fistula from returning. Contact your doctor if you prefer not to digitate the wound or cannot reach the area. 
    • Support your feet with a step stool when you sit on the toilet. It helps flex the hips and places the pelvis in a squatting position. This can make bowel movements easier after surgery.
    • Try lying on your stomach with a pillow under your hips to reduce swelling.
    • Avoid tight clothing.
    • Call your doctor if your temperature is over 101.0 degrees.

    Diet To Follow During Recovery

    There is a slight chance for recurrence of anal fistula after surgery. Therefore, having a good diet plan and following healthy lifestyle habits are essential to ensure quicker healing. Consuming very little fiber in food is usually the cause of experiencing fistula. To enhance recovery after surgery, it is important to consume food that is rich in fiber. Some of the fiber-rich foods are as follows:

    • Cereals: Brown rice, Oatmeal, Ragi, Quinoa.
    • Pulses: Chickpeas, Kidney beans, Soybeans.
    • Vegetables: All gourds-bitter gourd, Snake gourd, Cauliflower, Spinach, Carrot, Bottle gourd, Ivy gourd, Green leafy vegetables.
    • Fruits: Custard Apple, Pears, kiwi, Grape, Watermelon, Papaya, Banana, and Apple.
    • Milk and Milk products: Skim milk, Paneer, Cottage Cheese, Cream, Yoghurt.
    • Meat, Fish, and Egg: Tuna, Salmon, Herring, fatty fish.
    • Raw cabbage juice: Drink half a cup of raw cabbage juice daily to soften stools.
    • Water: Drink plenty of water, which helps clear and remove toxins and keeps you hydrated.

    Diet To Avoid During Recovery

    List of foods to avoid after fistula surgery includes the following:

    • Caffeine: Avoid caffeine after surgery because it can result in dehydration. Some food items that include caffeine are coffee, tea, chocolates, etc.
    • Alcohol: This can also cause dehydration which can lead to hard stools.
    • Large and heavy meals: Avoid large and heavy meals. This can ease digestion and gives relaxation to the intestines.
    • High-fat dairy products: Double-fat milk, cheese, and other heavy milk and cream products should be avoided as they are harder to digest and can hamper recovery after fistula surgery.
    • Fast food: Fast food consumption should be toned down after anal fistula surgery. Furthermore, food items made with flour (maida) should be curbed until symptoms have subsided.
    • Spicy foods: Spicy foods that are quite difficult to digest should be avoided after surgery. Try consuming simple homemade food. Anything that aggravates the stomach will not be a great option to try in your recovery phase. 
    • Fried foods: Foods that are hard to digest and have low nutrition content should be strictly avoided.
    • Salty foods: Bloating can make excretion very tedious. Consuming salty foods will not only make you feel bloated but may also aggravate your stomach and cause irritable bowel syndrome.
    • Packaged foods: Pre-packed foods such as chips, fries, and frozen items should be avoided after surgery. They barely have any nutritional content and are highly harmful to health.

    Medications To Take During Recovery

    Patients may need to take different types of medication for a short period after their fistula surgery to manage or prevent pain, constipation, and infections. Simple analgesics like paracetamol or ibuprofen are excellent choices to reduce pain after the anesthetic has worn off. 

    It is good to avoid opioid medications like codeine, which can cause constipation. Also, never take over-the-counter pain medication if you are on prescription medication for pain. 

    Pain medication should be taken a quarter of an hour before going to the toilet to ease defecation pain during this period. Prophylactic antibiotics may be used to lower the risk of infection in the area before and after the procedure. 

    Exercises To Do For A Better Recovery

    These exercises can help in proper recovery after your anal fistula surgery.

    Pilates

    Pilates can help improve posture, strengthen core muscles, and reduce stress in the anal area. Incorporate exercises like plank, toe taps, single leg stretch, etc., into your daily routine to keep your body strong after surgery.  

    Walking

    Walking keeps your body moving and reduces pain associated with surgery. Aim for at least 30 to 40 minutes of walking daily, or break it into smaller chunks throughout the day.

    Swimming

    Swimming is one of the best exercises during recovery as it helps reduce inflammation and boost mobility. Just be sure to avoid strokes that pressure your lower back or abdomen.

    Yoga

    Certain yoga poses can help relieve pain after surgery by increasing circulation to the affected area and improving posture. Try some of the poses, like pigeon pose, seated twist pose, etc., daily to keep your body healthy.

    Core Exercises

    A strong core can help relieve pain associated with surgery. Try doing planks, side planks, crunches, and other core exercises for a few minutes each day to keep your core muscles firm.

    Stretching

    Stretching the lower back, legs, and hips can enhance flexibility and ease tension in the affected area. It is vital to warm up before stretching and focus on deep breathing during the exercises.

    Kegel exercises

    Kegel exercises help strengthen the muscles around your anus, improving circulation and reducing strain in the area. Practice squeezing and relaxing the muscles several times daily to keep them strong and healthy.

    Precautions To Be Taken After Fistula Surgery

    Do not perform strenuous activities

    You can start doing regular work after a few days of surgery. Yet, you should avoid heavy lifting or physical activities such as outdoor sports for a considerable period. 

    Do not ignore bloating after fistula surgery

    If there is bloating and you can’t pass gas due to a complication around the surgical wound, consult your doctor if it persists. Never ignore such complications if you want to enhance your recovery without further complications.

    Consult a doctor in case of nausea

    If you encounter feelings of vomiting or nausea, a high fever that stays for hours, or continuous bleeding from the surgical wound, contact your doctor instantly instead of taking care of the wound yourself.

    Consult your healthcare provider or seek immediate medical care if:

    • You have pain that does not improve after taking pain relievers or other medications.
    • You have wound or incisional problems (increased pain, discolorations, or concerns regarding infection).

    • You have a fever greater than 100.5 F for 24 hours.
