  • What is Stage 2 Gyno?

Stage 2 or Grade 2 gynecomastia refers to a moderate level of breast enlargement in males. Gynecomastia is a condition where glandular breast tissue in men becomes more prominent than usual. In Stage 2, the breast enlargement is more noticeable and may have a slight sagging appearance. This stage typically involves both glandular tissue and excess fat accumulation in the chest area.

Even though the growth of the breast tissues is not prominent at stage 2, it is noticeable through clothes and often contributes to psychological and social distress. To get rid of the man-boobs, most patients request to undergo surgery called male breast reduction surgery.

As the tissues are exclusively present underneath or around the areola, the surgery tends to provide favorable results in terms of chest contours. To learn more about the gynecomastia treatment procedure, call Pristyn Care and talk to our expert doctors.

How is Grade 2 Gynecomastia Diagnosed?

Diagnosing Grade 2 gynecomastia, like any other stage, involves a combination of medical history, physical examination, and sometimes additional tests. The diagnosis usually involves the following:

    • Medical History: The doctor will ask the patient about his/her medical history, including any medications taken on a regular basis (prescribed and over-the-counter), past illnesses, or any other condition that could cause hormonal imbalance and contribute to gynecomastia.
    • Physical Examination: After discussing the medical history, the doctor will palpate the chest to do a physical exam and assess the size and nature of the tissue growth. Normally, a doctor can tell the difference between breast glandular tissues and fat tissues by feeling them. But for confirmation, some imaging tests may be recommended.
    • Blood Tests: In all cases, blood tests are done to check hormone levels, androgen and estrogen. The results of blood tests can tell whether the imbalance involves excess production of the female hormone estrogen or low production of the male hormone androgens. The altered ratio is calculated to identify the potential cause.
    • Mammography or Breast Ultrasound: Imaging tests like mammography (more common in older men) or ultrasound may be used to evaluate the breast tissue and rule out breast cancer. This test is recommended if the doctor suspects the mass or tissue growth in the chest is abnormal.
    • Biopsy (Rare): A tissue biopsy is only recommended in rare cases to confirm that the growth is not cancerous.

Once Grade 2 gynecomastia is confirmed, the doctor discusses the patient’s treatment options, including observation, medication cessation/adjustment, lifestyle changes, or surgery. The tests performed help to create a tailored treatment plan for the patient.

Are you going through any of these symptoms?

    Grade 2 Gynecomastia Treatment Options

The treatment for grade 2 gynecomastia will primarily depend on the underlying cause and whether symptom management is possible without surgery. The options include the following:

    • Observation: In cases where man boobs developed due to puberty or temporary hormonal fluctuations, the doctor may recommend a period of observation. This involves monitoring the condition over time to see if it resolves on its own. In teenagers, the condition usually resolves on its own. If the growth persists for 2 years or longer, further intervention may become crucial.
    • Lifestyle Changes: Sometimes, lifestyle modifications such as weight loss, exercise, and avoiding substances like alcohol or recreational drugs that can contribute to gynecomastia may be recommended. It may halt the progression of the condition, but the existing growth cannot be reversed.
    • Medications or Hormonal Therapy: As the primary cause of gynecomastia is hormonal imbalance, the surgeon may recommend hormone replacement therapy. There are medicines approved by USFDA that can treat the condition, but targeted therapy, such as selective estrogen receptor modulators (SERMs), aromatase inhibitors, progesterone therapy, anti-estrogen therapy, androgen therapy, etc., may be suggested for the patient. The effects of this therapy will vary, and it may not be suitable for all gynecomastia patients.
    • Surgery: Surgical intervention may be considered if the breast enlargement is persistent, causing discomfort, or significantly affecting self-esteem. There are different surgical options, including liposuction, gland excision, and a combination of these procedures called male breast reduction surgery.Liposuction is the preferred method of treatment in the case of pseudo-gynecomastia, where only fat tissues are present. Thus, liposuction can target them and remove them efficiently. Gland tissue excision is preferred in the case of true gynecomastia, where only breast glandular tissues are present. As in most cases, both gland tissues and fat tissues are present together, male breast reduction surgery is the ideal option to get the best results.

The choice of treatment for grade 2 gyno depends on the specific circumstances and patient’s preferences.

Benefits of Grade 2 Gynecomastia Treatment

Getting treatment for stage 2 gyno is beneficial for patients in the following aspects:

    • Improved Self-Esteem: The treatment can enhance self-confidence and body image, reducing feelings of embarrassment or self-consciousness that often accompany gynecomastia.
    • Psychological Well-Being: Treating gynecomastia can contribute to improved mental health, reducing feelings of anxiety or depression associated with the condition.
    • Health Benefits: Addressing gynecomastia at an early stage can also help identify and treat underlying hormonal imbalances or medical conditions contributing to the condition, potentially improving overall health.
    • Clothing Choices: Patients can wear clothes of their choice more comfortably and confidently, as they are no longer limited by concerns about their chest appearance.

Once the enlarged breast tissues are removed, they don’t grow back, and hence, the patients enjoy long-lasting results.

    Grade 2 Gynecomastia Surgery

Surgery for grade 2 gynecomastia is mostly an elective procedure as the enlargement is minimal. The procedure is called male breast reduction or reduction mammoplasty and involves the following steps:

    • General anesthesia or local anesthesia with IV sedation is done to numb the body and ensure the patient doesn’t feel any pain or discomfort during the surgery.
    • At first, liposuction is used to remove excess fat. The liposuction cannula is inserted into the chest to break down the fat tissues and remove them through suction.
    • At grade 2 gyno, direct excision of the glandular tissues is also necessary. The surgeon will make an incision around the areola and excise the breast tissues through that. The chest contour is then modified to create a more masculine appearance.
    • Once the tissues are removed, the incisions are closed with dissolvable sutures and covered with dressings or bandages.

The entire procedure will take around 40 to 60 minutes, and after that, the patient will be transferred to the recovery room. The nurses will monitor the patient’s vital until the effects of anesthesia wear off, and then the patient is moved to a private or shared ward, as chosen.

In most cases, the patient is discharged after a few hours of observation and given a detailed post-op care guide and follow-up schedule.

Potential Risks of Surgery for Stage 2 Gyno

Whether gynecomastia surgery is being performed at stage 2 or stage 4, the potential risks and complications remain the same. Some common risks include the following:

    • Infection can occur at the incision site
    • The incisions may heal abnormally, causing visible scarring.
    • Blood accumulation or hematoma may occur in the treated area.
    • Fluid accumulation or seroma may occur in the treated area.
    • The chest may appear asymmetrical after the surgery.
    • There may be some changes in nipple sensations, temporary or permanent.
    • The outcome of the surgery may not be satisfactory.
    • Allergic reactions to anesthesia may also occur.
    • Revision surgery may be needed due to contour irregularity.

Most of the risks and complications associated with gynecomastia surgery can be successfully avoided by an experienced surgeon.

Furthermore, the patient can also prevent post-op complications by adhering to the surgeon’s instructions for a safe and successful recovery.

Grade 2 Gynecomastia Surgery Cost in India

On average, grade 2 gynecomastia surgery cost in India is Rs. 40,000 to Rs. 70,000. It is only an estimated price range, and the actual cost of surgery may vary for each patient depending on the following factors:

    • The extent of tissues that need to be removed
    • The surgeon’s and anesthetist’s fees
    • Type of anesthesia used
    • Technique used
    • Hospital-related expenses
    • Pre-operative and post-operative tests
    • Post-op care, medications, compression garments, and follow-ups

The cost of gynecomastia surgery at all grades is calculated after accounting for the above-listed factors. To get a closer estimate for grade 2 gyno surgery cost, call Pristyn Care.

Results of Grade 2 Gyno Treatment

The results of Grade 2 gynecomastia surgery can vary from person to person, but the goal of the procedure is to achieve a more masculine chest contour by reducing excess breast tissue and fat. Some general things patients can expect in terms of results of gynecomastia surgery include the following:

    • Reduction in Breast Size: The main goal of the surgery is a noticeable reduction in breast size. The excess glandular tissue and fat that contributed to the enlarged breasts should be removed or reshaped properly to restore a masculine chest.
    • Improved Chest Contour: The surgery aims to create a flatter and more sculpted chest contour.
    • Enhanced Symmetry: The surgeon will work to improve the symmetry of the chest to make it appear equal. However, it should be noted that absolute perfection in symmetry may not always be attainable.
    • Minimized Areolar Size and Nipple Position: If the areolas were stretched due to gynecomastia, the surgery can also reduce their size and improve the nipple’s position and orientation.

To achieve the best possible outcome from grade 2 gyno surgery, the patient needs to follow the post-operative instructions given by the surgeon.

Recovery Timeline After Grade 2 Gyno Surgery

Gynecomastia surgery is a minimally invasive procedure. Due to this, the recovery after gyno surgery is quick and takes only around 2 to 4 weeks. Patients should keep in mind that the recovery period is crucial in achieving optimal results.

Thus, throughout this period, they need to follow the doctor’s instructions carefully.

The general timeline is given below:

    • In the First Week: There may be significant discomfort in the initial days, which will be managed through painkillers. The patient will be advised to take proper rest and avoid strenuous activities that could strain the chest. Besides this, the patient is given a compression garment that needs to be worn at all times in the first week to reduce the swelling and support the chest muscles.
    • Week 2-3:  Within the second week, the patient can resume normal activities like walking or climbing stairs, but strenuous activities still need to be avoided. The swelling and bruising resulting from surgery will start to subside during this period. The patient also needs to continue wearing the compression garment for the third week for at least 12 hours a day.
    • Week 4: By the fourth week, most patients can go back to their daily life routine. Still, certain precautionary measures need to be taken for the next week. This includes taking care of the incision scar and applying scar reduction creams to improve its appearance.

The results of gynecomastia surgery will start to become apparent as the treated area starts to heal.

Care Tips to Follow During Recovery

Some general care tips that patients should follow after stage 2 gyno surgery are given below:

    • Take the prescribed medications, including painkillers, antibiotics, anti-inflammatory medicines, etc., strictly as directed by the doctor.
    • The drain will be removed within 6 days, and until then, don’t touch it or try to clean it.
    • The arm motion might be limited for a week, and it’s important to keep restricting unnecessary movements.
    • Wear a compression vest as advised by the surgeon for 4 weeks to support the chest.
    • Sleep on the back in the first 3 weeks following the surgery.
    • The patient will be given permission to perform lower body workouts after 1-2 weeks but still has to avoid strenuous activities for a few more weeks.

The doctor will provide a detailed guide for post-op care to ensure a smooth recovery.
