What is Stage 3 Gyno?

Stage 3 or grade 3 is an advanced stage of breast enlargement in males. At this stage, the breast tissue growth is significant and often clearly visible, even through clothes. The nipples become large and puffy, almost similar to that of females, and the chest looks very similar to breasts.

The breast tissue extends over the chest fold, and the moderate excess skin creates a sight breast roll. As the skin has relaxed and is no longer able to compress, the chest loses the tight and round shape and has a more horizontal appearance.

The nipple also descends downward, and the internal tissue growth is also horizontal, with both fat and glandular tissues present underneath.

At this stage, the doctors suggest a combination of measures to prevent the growth of breast tissues. In most cases, surgery is recommended for the patients to treat gynecomastia at this stage effectively.

Grade 3 Gynecomastia Diagnosis and Evaluations

Typically, the doctor asks about the patient’s symptoms and examines the tissues in the chest area. In the physical exam, the doctor palpates the tissues to identify their composition and size. The doctor also takes a detailed medical history of the patient to identify the cause properly.

This includes gathering information about any medical condition the patient has, drugs or substance use, medications that are taken on a regular basis, etc. For further evaluations, the doctor may recommend the following:

  • Hormone and Blood Test: These tests are ordered to evaluate the hormone levels, especially testosterone and estradiol, in the blood.
  • Imaging Studies: For grade 3 gyno, imaging tests like mammography and ultrasound are important. They help to assess the composition of the breast tissues and rule out other conditions, especially breast cancer. They are also helpful to guide the treatment and create a plan according to the patient’s specific needs.
  • Biopsy: This test is only recommended if the doctor suspects that the tissue growth in the chest region is cancerous.

Besides these tests, the doctor also recommends a psychological assessment to evaluate the psychological impact of gynecomastia on the patient. This helps to determine the factors that may be influencing the decision to pursue treatment.

Are you going through any of these symptoms?

Treatment Options for Grade 3 Gynecomastia

For grade 3 gynecomastia, patients are usually recommended surgical treatment. The common options include the following:

  • Liposuction: This method of treatment is preferred for stage 3 gyno when mostly fat tissues are present. It involves using a liposuction cannula that breaks down the fat tissues and suctioning out the fat. Only tiny incisions are made to insert the cannula, which makes the procedure minimally invasive.
  • Excision Surgery: This is a common and effective solution for grade 3 gyno when only breast glandular tissues are present. It involves surgical excision of the tissues and skin to restore the masculine chest shape and contour. In some cases, it also involves nipple repositioning and resizing to give the chest a natural appearance.
  • Male Breast Reduction: Also known as reduction mammoplasty, this method is minimally invasive and combines both liposuction and excision techniques to remove the breast and fat tissues effectively.

The most suitable option is determined by the doctor after proper diagnosis and evaluation of the condition.

Grade 3 Gynecomastia Surgery

With the advancement of gynecomastia to stage 3, doctors usually have to modify the surgical treatment plan due to the progressive increase in skin laxity.

It means that the surgeon will have to remove the excess skin while removing the breast glandular tissues and fat tissues. The most suitable technique for that is male breast reduction surgery, which is a combination of liposuction and gland excision.

The procedure is carried out in the following steps:

  • Prior to taking the patient to the operating room(OR), the nurses will insert an IV line that will be used to administer fluids and medications. After that, the patient is taken to the OR.
  • In the OR, the anesthetist administers the anesthesia, and the patient’s chest area is cleaned and sterilized
  • Once the patient is numb, the surgeon will make an incision around the nipple in the areolar region. The incision size is usually small.
  • Then, a liposuction cannula is inserted into the chest area through a tiny incision to break down the fat deposits. Then, suction is used to extract those fat tissues.
  • After that, a scalpel is used to excise the glandular tissues carefully. If necessary, the excess skin around the chest area is also excised to improve the contours and create a more sculpted chest.
  • When the required changes are done, the incisions are closed with stitches or dissolvable sutures. The treated area is covered with dressings and compression bandages, and the patient is transferred to the observation room.

The entire procedure takes around 40 to 60 minutes. After surgery, the patient’s vitals are monitored, and once the effects of anesthesia wear off, he is transferred to the recovery room.

As male breast reduction surgery or gynecomastia surgery is minimally invasive, it is performed on an outpatient basis. That means the patient is discharged from the hospital within a few hours after ensuring his vitals are stable.

During discharge, the patient is given a detailed recovery guide and a follow-up schedule for a smooth and successful recovery.

Benefits of Minimally Invasive Gynecomastia Surgery

Minimally invasive male breast reduction offers several benefits compared to traditional gynecomastia surgery. Some of the key advantages are given below:

  • Smaller Incisions: As the technique is minimally invasive, it involves smaller incisions, which means less noticeable scarring.
  • Minimal Tissue Trauma: These procedures are less traumatic to the surrounding tissues, leading to less pain, swelling, and bruising after surgery.
  • Faster Recovery: Patients generally experience a quicker recovery after male breast reduction compared to traditional surgery. They can also return to their normal activities sooner.
  • Reduced Risk of Complications: The risk of complications such as infection, bleeding, and poor scarring is typically lower in minimally invasive procedures due to reduced tissue trauma and minimal exposure to external contaminants.
  • Preservation of Nipple Sensation: Minimally invasive techniques, like liposuction-assisted gynecomastia surgery, are designed to preserve nipple sensation and blood supply to the nipple-areolar complex.
  • Improved Cosmetic Outcomes: The minimally invasive technique provides excellent cosmetic outcomes, with a more contoured and natural-looking chest without major scarring.
  • Outpatient Procedure: As male breast reduction is performed on an outpatient basis, the patient can return home on the same day of the procedure.

Potential Risks & Complications of Grade 3 Gyno Surgery

Although grade 3 gyno surgery is safe, there are some potential risks and complications associated with the procedure. Some of them are given below:

  • Infection: An infection can occur in the incision site, which will be treated with antibiotics. However, if an abscess forms around the treated area, drainage may be required with medicines.
  • Bleeding or Hematoma: Typically, bleeding is normal after surgery, but in some cases, excessive bleeding may occur, or the blood may accumulate around the surgery site. If this happens, drainage may be required, and medicines will be prescribed to stop the bleeding. The compression vest is prescribed to patients to prevent these complications.
  • Asymmetrical results: In patients with grade 3 gynecomastia, it can be slightly difficult to excise the same amount of skin and tissue from both breasts. Due to this, there is a risk of asymmetry between the chest, for which the patient may require revision surgery.
  • Changes in Nipple Sensation: During surgery, there is a high chance of temporary or permanent nerve damage, which can affect the nipple sensation, which may or may not improve over time.
  • Seromas: It is the accumulation of fluid in the surgical site, which may need aspiration. Typically, drainage tubes are placed around the treated area to prevent fluid accumulation.
  • Skin Irregularities: As grade 3 gyno surgery involves skin removal, there is a chance that the chest contour won’t be regular. However, this risk usually occurs when liposuction is not used for chest contours.
  • Anesthesia Risks: In some cases, patients may experience reactions to anesthesia, such as aspiration, respiratory issues, etc.

In some cases, there are also possibilities of patients developing DVT or gynecomastia recurrence, but these are rare. Moreover, the surgeon usually takes the necessary measures to mitigate these risks and ensure the best possible outcome.

Recovery & Results After Stage 3 Gyno Surgery

The recovery after stage 3 gyno surgery takes around 2 to 4 weeks. The final results will appear only after the healing is complete, which is most likely to take longer than a month. The general timeline for recovery after grade 3 gynecomastia treatment is given below:

  • First Week: After the surgery, the patient may experience slight discomfort, swelling, bruising, and soreness. The doctor will advise wearing a compression garment 24/7 in the first week to reduce the swelling and support the chest muscles. Most patients can resume light activities and even return to work within a week, but strenuous activities need to be avoided for a few more weeks.
  • Second and Third Week: In the second week, the patient will start to notice improvements in the chest contour, but there will still be swelling, which can mask the final results. Intense physical activities still need to be avoided as they can strain chest muscles.
  • Fourth Week: By the fourth week, most of the swelling will be gone, and the patient will have a good idea of what will be the final outcome. There will be scarring, for which the doctor will suggest using silicone gel, sheets, or other scar reduction cream.

In the upcoming weeks, the chest contour will continue to improve, and the final results will become apparent. However, to get the desired results, the patient will have to incorporate exercises and eat a balanced and healthy diet. It should also be noted that the recovery timeline and results will vary for each patient.

While some people may see the results within a month, others may have to wait for 3-4 months. It’ll be best to talk to the doctor regarding what to expect in terms of recovery and results.

Post-op Care Tips After Gynecomastia Surgery

The recovery period is crucial to get the desired results after gynecomastia surgery. Some post-op care instructions given by the doctor include the following:

  • Take the prescribed medications, including painkillers, antibiotics, anti-inflammatory medicines, etc., as directed by the doctor.
  • Wear compression garments, such as a surgical vest or compression binder, as instructed by the doctor for the specified period of time.
  • Take proper rest during the first few days after surgery and avoid all activities that can strain the chest area.
  • Keep an eye on the incision site and look out for any signs of infection, such as excessive bleeding, discharge, high fever, increasing swelling, etc. If these symptoms arise, contact the doctor immediately.
  • Use cold compresses during the initial days to reduce the swelling and bruising in the chest area.
  • Eat a balanced diet that can aid in the healing process and provide ample nutrition to support tissue repair and recovery. Also, drink plenty of water to keep the body hydrated.
  • Avoid smoking and drinking alcohol, as they can hinder the healing process and increase the risk of complications.
  • Attend all the scheduled post-surgery follow-ups without fail so that the surgeon can monitor the progress, remove stitches (if required), adjust medicines dosages, and address complications, if any.
  • Use scar creams or silicone gel as directed to improve the scar’s appearance.

Throughout this period, the patient needs to be diligent in following the care instructions.

Common FAQs Around Grade 3 Gynecomastia

Is grade 3 gynecomastia surgery cost covered by insurance?

Yes, in most cases, grade 3 gynecomastia surgery cost is covered by health insurance. This is mostly because, at this stage, the enlargement is significant, and it causes physical discomfort in males.

Due to this, the treatment is considered medically necessary, and insurance providers approve the claim request after validating the clinical documents. To learn more about insurance coverage, it’ll be best to talk to the insurance provider.

Can medication alone effectively treat grade 3 gynecomastia?

No, medication may be considered as part of the treatment plan, but it is generally more effective for early-stage gynecomastia. Grade 3 gynecomastia often requires surgical intervention for optimal results.

Will I have visible scars after grade 3 gynecomastia surgery?

Scarring is inevitable with any surgical procedure, but with male breast reduction surgery, plastic surgeons take a minimally invasive approach to reduce scarring. The incisions are also placed strategically so that once they heal, the scars are not prominent. Moreover, with proper care, scars tend to fade over time.

Is it possible for grade 3 gynecomastia to return after surgery?

While surgery provides long-lasting results, there is a minimal risk of recurrence, especially if underlying hormonal imbalances are not addressed. That’s why patients are often advised to take regular follow-ups and take medicines to maintain their hormonal balance.

Can I combine other procedures with grade 3 gynecomastia surgery, like liposuction or abdominal contouring?

Yes, it’s common to combine procedures for a more comprehensive transformation. You can consult with an experienced plastic surgeon to discuss your goals and the possibility of combining surgeries.
