  • What is Stage 3 Gyno?

    Stage 3 or grade 3 is an advanced stage of breast enlargement in males. At this stage, the breast tissue growth is significant and often clearly visible, even through clothes. The nipples become large and puffy, almost similar to that of females, and the chest looks very similar to breasts. 

    The breast tissue extends over the chest fold, and the moderate excess skin creates a sight breast roll. As the skin has relaxed and is no longer able to compress, the chest loses the tight and round shape and has a more horizontal appearance.

    The nipple also descends downward, and the internal tissue growth is also horizontal, with both fat and glandular tissues present underneath.

It involves using a liposuction cannula that breaks down the fat tissues and suctioning out the fat. Only tiny incisions are made to insert the cannula, which makes the procedure minimally invasive. 
    • Excision Surgery: This is a common and effective solution for grade 3 gyno when only breast glandular tissues are present. It involves surgical excision of the tissues and skin to restore the masculine chest shape and contour. In some cases, it also involves nipple repositioning and resizing to give the chest a natural appearance. 
    • Male Breast Reduction: Also known as reduction mammoplasty, this method is minimally invasive and combines both liposuction and excision techniques to remove the breast and fat tissues effectively. 

    The most suitable option is determined by the doctor after proper diagnosis and evaluation of the condition. 

    Grade 3 Gynecomastia Surgery

    With the advancement of gynecomastia to stage 3, doctors usually have to modify the surgical treatment plan due to the progressive increase in skin laxity.

    It means that the surgeon will have to remove the excess skin while removing the breast glandular tissues and fat tissues. The most suitable technique for that is male breast reduction surgery, which is a combination of liposuction and gland excision. 

    The procedure is carried out in the following steps:

    • Prior to taking the patient to the operating room(OR), the nurses will insert an IV line that will be used to administer fluids and medications. After that, the patient is taken to the OR. 
    • In the OR, the anesthetist administers the anesthesia, and the patient’s chest area is cleaned and sterilized
    • Once the patient is numb, the surgeon will make an incision around the nipple in the areolar region. The incision size is usually small. 
    • Then, a liposuction cannula is inserted into the chest area through a tiny incision to break down the fat deposits. Then, suction is used to extract those fat tissues. 
    • After that, a scalpel is used to excise the glandular tissues carefully. If necessary, the excess skin around the chest area is also excised to improve the contours and create a more sculpted chest. 
    • When the required changes are done, the incisions are closed with stitches or dissolvable sutures. The treated area is covered with dressings and compression bandages, and the patient is transferred to the observation room. 

    The entire procedure takes around 40 to 60 minutes. After surgery, the patient’s vitals are monitored, and once the effects of anesthesia wear off, he is transferred to the recovery room.

    As male breast reduction surgery or gynecomastia surgery is minimally invasive, it is performed on an outpatient basis. That means the patient is discharged from the hospital within a few hours after ensuring his vitals are stable.

    • Bleeding or Hematoma: Typically, bleeding is normal after surgery, but in some cases, excessive bleeding may occur, or the blood may accumulate around the surgery site. If this happens, drainage may be required, and medicines will be prescribed to stop the bleeding. The compression vest is prescribed to patients to prevent these complications. 
    • Asymmetrical results: In patients with grade 3 gynecomastia, it can be slightly difficult to excise the same amount of skin and tissue from both breasts. Due to this, there is a risk of asymmetry between the chest, for which the patient may require revision surgery.

    • First Week: After the surgery, the patient may experience slight discomfort, swelling, bruising, and soreness. The doctor will advise wearing a compression garment 24/7 in the first week to reduce the swelling and support the chest muscles. Most patients can resume light activities and even return to work within a week, but strenuous activities need to be avoided for a few more weeks. 
    • Second and Third Week: In the second week, the patient will start to notice improvements in the chest contour, but there will still be swelling, which can mask the final results. Intense physical activities still need to be avoided as they can strain chest muscles. 
    • Fourth Week: By the fourth week, most of the swelling will be gone, and the patient will have a good idea of what will be the final outcome. There will be scarring, for which the doctor will suggest using silicone gel, sheets, or other scar reduction cream.
