  • What is Grade 4 Gynecomastia?

The grade 4 gynecomastia or gyno is characterized by the excessive drooping of the breasts that creates a clearly visible fold in the chest region. At stage 4, the nipple also starts to become droopy.

The distinct and obvious difference between stages 3 and 4 is the presence of breast roll due to excessive breast glandular growth and fat deposition in the chest area.

Stage 4 gyno is the most advanced and severe form of enlarged breasts in males. When the condition advances to this stage, patients start to experience severe pain in the shoulder and back due to excessive breast tissue growth and suffer from emotional and mental challenges as well.

The treatment option for grade 4 gyno is mostly surgery combined with medical intervention to prevent its recurrence. To plan the surgery, get in touch with Pristyn Care and talk to our care coordinators.

Diagnosis & Evaluations of Stage 4 Gyno

Diagnosis of grade 4 gynecomastia typically involves the following:

    • Medical History: The doctor will take a detailed medical history and ask about symptoms, the duration of breast enlargement, any medications or supplements being taken, and any underlying medical conditions.
    • Physical Examination: The doctor will physically examine the chest to assess the size, shape, and texture of the breast tissue. The exam also helps to see if there are any signs of inflammation, tenderness, or breast discharge.
    • Blood Test: A complete blood test may be ordered to measure hormone levels, including testosterone, estrogen, and other related hormones that contribute to gynecomastia.
    • Imaging Tests: This involves tests like ultrasound or mammography. These are done to check the composition of the tissues and rule out malignancies. The results of these tests also help to plan the surgery.

Along with these tests, a psychological evaluation of the patient is done to determine if the individual has some body image issues, social distress, low self-esteem, or other concerns due to gynecomastia.

Are you going through any of these symptoms?

    Stage 4 Gyno Treatment Options

Treatment options for grade 4 gynecomastia mostly involve surgical intervention, including the following:

    • Gland Excision Surgery: Often referred to as mastectomy in males, this procedure involves surgical excision of the enlarged breast tissues. It is the standard and traditional procedure.
    • Liposuction: This technique is used to target the fat tissues in the chest area. It involves using a specialized cannula to break down the fatty tissues and extract them using suction. It is a minimally invasive procedure that is performed when enlargement in the chest area is caused mostly by fat tissues.
    • Skin Reduction or Excision: In grade 4 gynecomastia, excessive skin is present in the chest area. As the breast tissues start to grow, the skin expands with the growth. Hence, while removing the glandular tissues, the surgeon also needs to excise the excess skin to reduce the breast roll and create a masculine chest contour.
    • Male Breast Reduction Surgery: This is a combined technique in which gland excision, liposuction, and skin reduction are done altogether to remove the breast, fat, and skin tissues effectively. It provides the best outcomes in terms of aesthetics and removes most of the breast tissues, leaving almost negligible chances of recurrence.

Besides surgery, doctors often recommend hormonal therapy or medications to maintain the balance. The most suitable approach for stage 4 gyno treatment is decided according to the patient’s personal preferences.

    Grade 4 Gynecomastia Surgery

Whether the patient has grade 1, grade 2, grade 3, or grade 4 gynecomastia, the most suitable option for treatment is male breast reduction surgery, which is commonly called gynecomastia surgery.

It is a minimally invasive procedure where the surgeon efficiently removes the excess breast glandular tissues, fat tissues, and extra skin to improve the chest contours.

The procedure of grade 4 gyno treatment is slightly more complex than at other stages. It is carried out in the following steps:

    • The patient is taken to the operating room, where an anesthetist will administer general anesthesia. If general anesthesia is not a viable option, local anesthesia with sedation will be used.
    • After the patient is numb, the surgeon will make incisions around the areola, beneath the chest, or in the underarm region. In grade 4 gyno treatment, the incisions are usually slightly larger as an extensive amount of tissue needs to be removed during the procedure.
    • The liposuction cannula is inserted into the chest first to break down the fat tissues and extract them using suction.
    • After that, a scalpel is used to excise the breast glandular tissues carefully. It should be noted that all glandular tissues are not removed during the procedure, as excising them can increase the risk of damaging the pectoral muscles.
    • When the internal tissues are removed, the surgeon will excise the skin tissues to reshape the chest. The nipple is also repositioned to provide more natural aesthetics to the chest.
    • Once the appropriate contours are achieved, the surgeon will close the incisions using stitches or dissolvable sutures. The chest is then covered with dressing and compression bandages.

After this, the patient is kept under observation until the effects of anesthesia wear off.

Benefits of Treatment at Grade 4 Gyno

When gynecomastia has advanced to grade 4, surgical treatment can provide various benefits to the individual, both physical and psychological. Some key benefits include the following:

    • Improved Chest Contour: The goal of gynecomastia surgery is to restore the masculine chest contour. At stage 4 gyno, the surgery will dramatically improve the contours as a significant amount of tissues will be removed during the procedure to create a flatter and firmer chest.
    • Enhanced Self-Confidence: In most males, when the chest starts to appear like female breasts, it gives rise to self-esteem and confidence issues. So, when the surgery successfully restores the normal chest appearance, they regain their confidence and self-esteem and have a positive body image.
    • Physical Comfort: Enlarged male breasts often cause physical discomfort in the back, neck, and shoulders. Furthermore, the individual may have difficulties in performing physical activities. Surgery can provide relief from such discomfort and allow the individual to lead a more active lifestyle.
    • Elimination of Psychological Distress: Grade 4 gynecomastia can lead to psychological distress, including anxiety and depression. The male breast reduction surgery can significantly reduce and, over time, eliminate these emotional challenges, improving overall mental well-being.
    • Long-Lasting Results: Once the glandular tissues are removed from the chest, there are almost negligible chances that an individual will develop gynecomastia again as long as the hormonal levels are kept in check.
    • Clothing Options: After surgery, individuals often find it easier to wear a wider range of clothing styles without worrying about concealing or hiding their chest. This can provide them with a wider range of choices of clothes with proper comfort.
    • Improved Posture: The breast tissue growth often contributes to poor posture in males. By removing the breast tissues, surgery helps individuals achieve better posture and prevent future issues with their neck, shoulder, and spine.

Gynecomastia surgery is a highly customizable treatment where the surgeons can tailor the procedure according to the specific needs and goals of the patient to ensure the best possible outcomes.

Potential Risks and Complications of Gynecomastia Surgery

Surgery for grade 4 gyno is more extensive than at any other stage. The potential risks and complications that may develop during and/or after the surgery include the following:

    • Infection: There is a risk of infection at the surgical site, which may lead to oozing, blood discharge, pain, high fever, persistent swelling, etc. If any of these symptoms are present, it’s important to seek help right away.
    • Bleeding: In some cases, the patient may bleed excessively during or after the surgery.
    • Scarring: At grade 4, the surgery is very extensive, and the incision size is comparatively larger. Due to this, the scar may be more clearly visible in the initial days. But after healing, the scar is likely to fade away.
    • Asymmetry and Skin Irregularities: Achieving proper symmetry of the chest at grade 4 can be quite difficult, and some degree of asymmetry and irregularity may persist after surgery. In some cases, patients may need additional procedures.
    • Hematoma and Seroma: There is a possibility of blood (hematoma) or fluid (seroma) accumulation in the treated area. If so, drainage or aspiration may be required to remove the blood or fluid.

Besides these, patients may experience anesthesia-related complications, such as allergic reactions, respiratory issues, etc., and other problems, such as delayed healing, unsatisfactory results, blood clots, etc.

Generally, the surgeon will take the necessary steps before the procedure to mitigate the risks during surgery. To reduce the chances of post-op complications, the doctor will give detailed care instructions.

Recovery and Results After Grade 4 Gynecomastia Surgery

The recovery after grade 4 gynecomastia surgery takes around 2 to 4 weeks. In some patients, the duration of recovery may be longer. The general timeline will be as below:

    • Immediately After the Surgery: On the first day, the patient will spend some time in the recovery room and will have bruising, swelling, and redness around the chest. The surgical site will be covered with bandages and compression garments. The doctor will advise the patient to take proper rest for the first few days.
    • Week 1 and 2: In the first and second weeks, the swelling and bruising will be most significant. The patient may be able to resume basic activities but still need to avoid strenuous activities. If stitches were used, they will be removed within 10-14 days after surgery, and the doctor will adjust the medication dosages accordingly.
    • Week 3 and 4: By the third and fourth weeks, the patients can resume normal activities, and the swelling will continue to subside. At this point, the chest appearance will become clearer, and it’ll appear more defined. The incision will also start to heal at this point.

For the next few months, the patient will be advised to use scar reduction creams, silicone sheets, or gels to help improve the scar’s appearance. To get the desired results, the patient will have to diligently follow the post-op care guidelines.

Post-op Care Tips After Gynecomastia Surgery

As mentioned, the recovery period is crucial to get the desired results after gynecomastia surgery and reduce the risk of post-op complications. Some common care instructions that patients should follow are given below:

    • Keep the incision site clean and dry until the stitches are removed, or they dissolve on their own. Change the bandages as directed by the surgeon.
    • Avoid swimming pools or hot tubs until the surgeon approves, as submerging the incisions in water can increase the risk of infection.
    • Wear a compression garment as directed to reduce the swelling and support the chest muscles during the healing process.
    • Take the prescribed pain medications, antibiotics, anti-inflammatory medicines, etc., as directed by the doctor. Don’t take any over-the-counter medications without consulting the doctor.
    • Keep physical activities limited during the first 2 weeks and resume activities after getting approval from the doctor.
    • Eat a healthy and well-balanced diet to support healing, and drink plenty of water and other drinks to flush out toxins from the body and keep it hydrated.
    • Refrain from smoking and alcohol consumption during the recovery period, as they can interfere with the healing process.
    • Avoid sun exposure until the scar has healed completely. Direct sun rays can cause hyperpigmentation or darkening of the skin. Discuss scar care with the surgeon after the incision has healed.
    • Attend all the follow-up appointments as scheduled by the surgeon so that the recovery can be monitored properly.
    • Keep an eye out for the signs of complications, including fever, persistent swelling, blood discharge, immense pain, etc., and report them to the doctor immediately.

It is important to stay in touch with the doctor throughout the recovery period and get proper advice regarding most activities for a smooth and successful recovery.
