  • Recovery After Hip Replacement Surgery

    The recovery time after hip replacement surgery can vary depending on the type of surgery and the individual. Your recovery will begin within 24 hours after surgery. However, full recovery from a hip replacement can take three to four months. Most hip replacement patients start to walk with a walker or crutches within a day or two after surgery. As the days advance, the patients can increase the distance and frequency of walking. 

    The new hip joint can ease pain and increase the hip’s range of motion. High-impact activities, like running or playing basketball, might be too stressful on the artificial joint after surgery. But in time, people can indulge in lower-impact activities, like swimming, golfing, and bicycle riding.

    Medications To Take During Recovery

    After hip replacement surgery, your doctor will advise some medications to control your pain. Many medicines are available to help control pain, like opioids, non-steroidal anti-inflammatory drugs (NSAIDs), local anesthetics, etc. Medications can help you feel more comfortable, allowing you to start moving sooner, regain your strength, and recover from surgery faster. Some of the medications to take after hip replacement surgery include the following:

     

    • Opioid Analgesics- Opioids analgesics are used for moderate to severe pain. When taken as prescribed, they can be effective for managing short-term pain after hip replacement surgery. In most cases, oral (PO), intramuscular (IM), or intravenous (IV) opioids will be prescribed after hip replacement surgery. 
    • Non-steroidal Anti-inflammatory Drugs- Non-steroidal anti-inflammatory drugs (NSAIDs) reduce inflammation and soreness and are used alone for mild to medium pain. To manage severe pain after surgery, NSAIDs are often used in combination with opioids. Some examples of NSAIDs include ibuprofen, aspirin, and naproxen.
    • Acetaminophen- Like NSAIDs, acetaminophen may reduce pain after hip replacement surgery. It is often combined with opioid medicine in tablet form. Acetaminophen eases mild to moderate pain by elevating the body’s overall pain threshold. 
    • Tramadol- Tramadol relieves pain through two different ways. In one way, tramadol works in the brain and spinal cord to change how the body senses pain. However, tramadol also works similarly to some antidepressant medications by interfering with regulating certain neurochemicals (serotonin and norepinephrine). When the amount of these chemicals changes, it becomes difficult for pain messages to be relayed from one nerve cell to the other. Hence, it reduces the amount of pain you feel.
    • Local Anesthetics- Local anesthetics block pain in a small area of the body. These medicines block the pain signals that travel along the nerves to the brain. Also, they do not cause the side effects of constipation, or breathing problems that you get with opioids. For a better recovery after hip replacement surgery, the most commonly used local anesthetics include lidocaine, and ropivacaine.

    When To Call Your Healthcare Provider After Hip Replacement Surgery?

    Call your healthcare provider immediately if you experience signs of hip replacement dislocation, including:

    • Intense pain in the hip and groin
    • A popping sound when you move
    • Difficulty walking or an inability to walk
    • Inability to move the hip joint
    • Noticing that the leg with the prosthetic hip is suddenly shorter than the other

    Post-Operative Care After Hip Replacement Surgery

    Some of the tips to follow after hip replacement surgery include the following:

    • Take your medication as advised by your doctor to make the pain more manageable.
    • The swelling in the leg may persist for three months or more after surgery. If the leg is very swollen, resting on the bed for an hour or so will help. Also, you may ice your thigh to reduce the swelling. You may use crushed ice, a gel pack, or a pack of frozen peas, which must be wrapped in a damp towel or tea towel before being placed on your thigh. 
    • Avoid bending your hip more than 90° during any activity.
    • Avoid twisting your hip.
    • Do not apply any pressure to the wound in the early stages of recovery (try to avoid lying on your side)
    • Try not to sit in the same position for more than 40 to 50 minutes. Also, keep your feet about 6 inches (15 centimeters) apart while sitting. 
    • When getting up from a chair, slide toward the edge of the chair, and use the arms of the chair or the walker for support. 
    • Do not force your hip or do anything that makes it feel uncomfortable.
    • Avoid low chairs and toilet seats. 
    • Be extra careful to avoid falls in the first few weeks after your hip replacement surgery, as this could damage your hip.
    • Take extra care on stairs and in the kitchen and bathroom, as these are places where people can have accidental falls.
    • Do not squat or reach for anything while showering. Also, use a shower sponge with a long handle for washing. Have someone change the shower controls if they are hard to reach. 
    • Get into the car from street level and not from the doorstep. Also, before entering a car, ensure you can slide easily on the seat material.
    • A special abductor pillow may be used to keep your hip in the proper alignment.
    • Wear shoes with nonskid soles. Avoid wearing slippers as they can make you fall. 
    • Lose weight after surgery, if required. You’ll only need to do this if you carry extra weight or have been diagnosed with overweight or obesity.

    • Alcoholic beverages – Alcohol can cause dehydration and impaired wound healing.
    • Caffeine drinks – Caffeine can cause dehydration due to its diuretic properties. Therefore, it is better to avoid caffeine while recovering from hip replacement surgery.

    Exercises To Do After Hip Replacement Surgery

    People recovering from hip replacement surgery should do hip-strengthening exercises daily. Below are some hip-strengthening exercises that doctors commonly recommend to hip replacement patients. Patients may be advised to do 10 or 15 repetitions of each exercise a few times daily.

    • Quad squeezes- The quadriceps muscles are found at the front of the thigh and help support hip joints. Quad squeezes can strengthen these muscles without putting strain on the hips.
    • Ankle pumps- Ankle pumps boost blood flow in the leg and strengthen the lower leg, which can help support the hip.
    • Gluteal sets- The gluteal set is an exercise that can help strengthen the muscles in the buttocks and help you regain range of motion after your surgery.

    • Heel slides- This exercise engages both the quadriceps muscles and the gluteal muscles. In addition, it also engages the quadriceps and hamstring muscles and will help enhance the patient’s range of motion.
    • Hip abductions- Abduction exercises need moving a limb away from the body. This exercise can help stabilize the pelvis and facilitate a normal walking gait.
    • Knee Extensions- Knee extensions can help strengthen the quadriceps and improve knee and hip flexibility.
