What is Renal Concretion?

Renal concretions are hard formations or stones that form in the kidneys. This condition can affect individuals of all age groups but most commonly occurs in individuals above 30 years of age. These stones are made up of different substances, including salt, uric acid, calcium, etc, and typically vary in size and number. Renal concretions are divided into the following types:

  • Calcium oxalate stones: These are the most common type of stones that form as a result of high levels of calcium or oxalate in the urine and low levels of citrate. 
  • Calcium phosphate stones: These are a less common type of calcium stone, and the cause of calcium phosphate stones isn’t known yet. But, they are mostly linked to a high urine pH.

TREATMENT FOR SMALL RENAL CONCRETIONS 

 In the case of small-sized kidney stones, the doctor may suggest some conservative treatments, including the following: 

  • Pain relieving medication: If your doctor suspects you have small stones that can pass naturally, they may suggest watchful waiting. But, since passing stones can cause some discomfort, they may recommend some pain relievers to help manage the pain. Some commonly suggested pain relievers include ibuprofen and naproxen sodium. 
  • Medical therapy: To help the stones pass easily, the doctor may also suggest medications known as alpha-blockers. These medicines help relax the muscles in the ureter and help the kidney stones pass more easily and without causing any pain or discomfort. 

Treatment for Medium To Large Renal Concretions

Extracorporeal Shock Wave Lithotripsy or ESWL: This is a non-invasive procedure for kidney stone removal that uses shock waves and X-ray or ultrasound to locate the stones, break them, and help them pass through the urine. The doctor administers spinal anesthesia before the surgery. The doctor then makes the patient lie on a water-filled cushion or tube to prevent damage to the surrounding organs. Then, imaging tests like X-ray or ultrasound are used to locate the stones. Once located, the stones are fragmented with the help of sound waves. After fragmenting the stones, they are allowed to flush out of the body on their own. This procedure only takes 35 to 45 minutes and does not involve any incisions. 

SURGERY

Ureteroscopic lithotripsy or URSL: This is a commonly performed procedure for kidney stone removal. This procedure is generally performed to extract stones that are small to medium in size. It is performed in the following steps: 

  • The doctor administers anesthesia (spinal or general) to the patient. 
  • Then, a long fiber-optic scope called a ureteroscope is inserted into the ureteral passage through the urethra.
  • Alongside, the surgeon uses an X-ray to locate the stones exactly.  
  • After locating the stone, the surgeon uses a stone basket to collect the stones and pulls it out of the system. 
  • In the case of large-sized stones, the surgeon may use a laser to break the stones and allow them to pass through the system naturally. 
  • The surgeon may place ureteral stents toward the end of the procedure to expand the passage of the ureter and allow stones to pass conveniently. 

Retrograde Intrarenal Surgery (RIRS): This minimally-invasive surgery involves no incisions or holes and is performed with the help of an optic endoscope. The surgery is performed in the following steps: 

  • The doctor administers general or spinal anesthesia to the patient to sedate them and let them be comfortable during the procedure. 
  • Then, a long and flexible tube (endoscope) is inserted into the urethra to reach the urine-collecting part of the kidneys. Meanwhile, the surgeon looks at the organs with the help of an X-ray, which helps them maintain precision.
  • Then the endoscope is then moved upwards in retrograde towards the kidneys in order to locate the stones. After locating the stones, a laser probe is used to break the kidney stones into smaller fragments.
  • The broken fragments are collected in a ‘stone basket’ and removed through the same passageway. 
  • The surgeons may use ureteral or double J stents for this surgery. They may be used before the surgery to dilate the ureter to allow the endoscope to pass easily during the procedure or after the surgery for 7 to 10 days to help with the healing process.

Tunnel Surgery or Percutaneous Nephrolithotomy (PCNL): This is another minimally-invasive surgery for renal concretion, which, unlike other surgeries, involves a small incision. This procedure is generally suggested to treat large renal concretions. It is performed in the following steps: 

  • The patient is sedated with general anesthesia before the surgery. This helps keep the patient asleep and at ease during surgery. 
  • Then, the surgeon makes a small incision in the flank area, i.e., the sides of the lower back area, before inserting a nephroscope with X-ray guidance to get the exact location of the stone. 
  • After locating the stone, the surgeon accesses the kidney’s urine-collecting system with the help of a thin needle. A guidewire is used in this step, which allows safe access for the nephroscope. 
  • After obtaining access, the surgeon begins the stone extraction process. Microforceps may be used to extract the stones. But, if the stones are too large in size, the doctor may use a laser to break the stones and then extract them from the body. 
  • Finally, after extracting all the stones, the surgeons use contrast to confirm that no stones or fragments are left in the ureter and then place a drainage catheter. A Dj stent may be placed to prevent swelling and allow easy drainage of urine.
