Diagnosis

If you have symptoms of a kidney stone, your urologist may perform the following diagnostic tests to confirm the presence of kidney stones and draft a treatment plan:

  • Blood and urine tests: Blood tests help assess the calcium and uric acid levels in the blood, which helps evaluate the health of the kidneys. A 24-hour urine collection test examines the components of the urine for stone-forming minerals and stone-preventing substances.
  • Imaging tests: Imaging tests like CT scans, abdominal X-rays, and ultrasounds help reveal the size and location of kidney stones.
  • Stone analysis: Often, large kidney stones are associated with small kidney stones. If you collect your urine and pass it through a strainer, you can obtain small renal stones and send them to the lab for analysis.

Surgery

For a kidney stone, you may need one of the following treatments:

  • ESWL (Extracorporeal Shock Wave Lithotripsy) is a common noninvasive treatment in which shock waves like X-rays or ultrasounds are used to break down stones in the urinary system, pancreas, or bile ducts.
  • RIRS (Retrograde Intrarenal Surgery) is an endoscopic surgery in which the surgeon uses a fiberoptic endoscope and a ureteroscope to locate and remove renal stones from kidneys or ureters.
  • Percutaneous Nephrolithotomy (PCNL) is performed to remove a kidney or upper ureter stones that are too large to be dissolved through ESWL or URSL.
  • In Ureteroscopic lithotripsy (URSL), the surgeon inserts a ureteroscope through the urethra to locate the stones, and then the stones are destroyed using shock waves. A urethral stent may be placed at the end of the surgery to keep the ureter open after the procedure.

Do all kidney stones require treatment?

The treatment for kidney stones depends entirely upon the size of the stone. Kidney stones that are smaller than 5 mm usually clear out on their own. Stones bigger than that often require medical or surgical  intervention. 

Will 18 mm kidney stones cause damage to my kidneys?

18 mm kidney stones can not pass through the urethra on their own. Therefore, medical intervention is requisite. If left untreated, these stones can block the ureters and can cause the kidneys to swell up. On top of that, they can also put you at an increased risk of infections. 

How are 18 mm kidney stones diagnosed?

If your doctor suspects kidney stones, he/she might recommend the following tests: 

Imaging tests- These tests will include abdominal ultrasounds, x-rays, or MRIs. They will help the doctor obtain 3D images of the abdomen, and determine the size and location of the stone. 

Blood tests- These tests will help the doctor look for calcium, phosphorus, uric acid, and electrolytes that may result in the progression of kidney stones.

Blood urea nitrogen (BUN) and creatinine test – This test will let the doctor determine the amount of nitrogen in the blood, and identify abnormalities in the kidney. 

Urinalysis – This test helps doctors assess the contents of urine to check for crystals, bacteria, or any other agents that can cause kidney stones. 

Are there any long-term consequences of having a kidney stone?

Untreated kidney stones can adversely affect your kidneys and may also lead to permanent damage. However, kidney stones that are treated in time do not cause any long-term consequences.

How do I reduce the risk of kidney stones?

Here are ways in which you can prevent kidney stones in the first place: 

  • Keep yourself hydrated
  • Eat a calcium-rich diet

What will the treatment for 18 mm kidney stones consist of?

An 18 mm kidney stone would almost always require surgery. The doctor might suggest any of the following surgeries to help you get rid of the kidney stone: 
ESWL (Extracorporeal Shock Wave Lithotripsy): This non-invasive procedure uses shock waves to break down kidney stones, and other parts. These stones usually clear out on their own after the procedure. 
RIRS (Retrograde Intrarenal Surgery): This is another non-invasive procedure, which is performed using a viewing tube called a fiberoptic endoscope. The stones are removed using forceps or fragmented using tiny lasers. The fragments then pass from the bladder with urine. 
URSL (Ureteroscopic Lithotripsy): This surgery is performed under anesthesia. In this procedure, a small telescope is passed through the urethra, into the ureter to extract a stone. The stone is usually broken with a laser, and the pieces are then removed with a grasping device. 
PCNL (Percutaneous Nephrolithotomy): In PCNL or tunnel surgery, the surgeon makes minor incisions on the lower back or flank area and uses a nephroscope to locate and fragment the stones. The broken stone is then flushed out.
