  • What is Nephrostomy?

Several people experience blockage in their ureters, which restricts their ability to pass urine. The blockage can occur due to kidney stones, trauma or injury to the kidneys or ureter, a congenital condition, or an infection. Nephrostomy is a procedure that involves the creation of an opening between the kidneys and the skin to allow urine drainage. This temporary solution for urine drainage helps prevent complications caused by ureteral blockage till normal function is restored. It also helps in preventing infections.

Doctors pass a nephrostomy tube i.e., a thin and flexible tube to the kidney through your skin. This tube is connected to a bag outside your body. The drained urine is collected in the bag. The bag is emptied through an in-built tap or other methods. This procedure is x-ray or ultrasound-guided, and also involves the use of a contrast dye.

    Diagnosis

Lab tests before nephrostomy are performed to look for the cause of obstruction in the ureters and rule out underlying conditions that can affect the procedure’s efficacy. Some commonly performed tests before nephrostomy are:

Blood & urine tests: These tests help doctors look for signs of infections in your blood and urine. They may also assess the levels of creatinine to assess your kidney function.

Ultrasound: An ultrasound uses sound waves to produce images of the kidney and ureters.

Bladder catheterization: In this test, the doctor injects a dye into your bladder using a catheter.. Then, an x-ray is used to obtain images of the kidneys, ureters, bladder, and urethra before and during urination. This test is performed when there’s a blockage of urine.

Renal muscular scan: In this test, the technician injects some radioactive material into the arm. A special camera tracks the radioactivity and produces images of the urinary system for the urologist to assess.

Cystoscopy: A cystoscope, i.e., A small tube with a camera and light, is inserted into the urethra to get a clear view of the urethra and bladder and look for the blockage and what might be causing it.

Magnetic resonance imaging (MRI): An abdominal MRI uses a magnetic field and radio waves to create detailed images of the organs and tissues that make up your urinary system.

How to prepare for Nephrostomy surgery?

Preparing for surgery can help you in more ways than one. It keeps you calm during the procedure and even helps in preventing complications that might occur during or after the surgery. Here’s how you can prepare for your nephrostomy surgery:

    • It is important to tell your doctor about the medicines you’re taking. Some medicines, such as blood thinners, can cause complications in the process.
    • If you’re allergic to anaesthesia, any medicine, or contrast dye, make sure you inform your doctor in advance.
    • If the doctor has suggested restrictions related to eating or drinking something, follow them ardently.
    • If you have a history of kidney disease, communicate it to your doctor.
    • Make sure you bring all your test reports with you. This will help the doctor plan the procedure accordingly.
    • If you have ever had excessive bleeding during surgery or an injury, or had a bladder or kidney surgery before, make sure your doctor has all the details.
    • Bring an attendant with you on the surgery day.

    Nephrostomy Procedure

The nephrostomy procedure is performed by a radiologist or a urologist. The procedure is generally performed on an outpatient basis, and the patient can return home on the same day. The procedure is 30 to 60 minutes long and is performed in the following order:

    • A cannula is placed in a vein in the arm of the patient to administer a drip of fluids, antibiotics, anaesthesia, and painkillers.
    • The patient is asked to lie flat on the tummy on an x-ray table.
    • When the patient is completely relaxed, some local anaesthesia is injected on the side of the back to numb the area.
    • Then, the doctor pushes a fine needle through the skin, into the kidney, along with a guide wire. The guide wire helps in placing the nephrostomy tube in the kidney.
    • Stitches are used to keep the tube intact. Advanced nephrostomy tubes come with a locking system.
    • The tube is then connected to a urine drainage/collection bag outside the body.

Recovery & Care After Nephrostomy Surgery

Most patients can resume their activities within a day or two of the procedure. However, some precautions are necessary. Your doctor will give you instructions on when to return for the removal procedure and how to care for your nephrostomy tube to prevent complications. Here are some after-care tips of nephrostomy procedures:

    • You will have to check your tube daily and empty the bag regularly.
    • You must check whether the dressing is dry and clean. If it’s unclean, you must get it replaced immediately to prevent infections.
    • You must inspect the colour of the urine on a daily basis. If you notice a change in the colour of the urine, immediately get in touch with your doctor.
    • Do not forget to examine the skin around the dressing for redness or rashes.
    • Empty your drainage bag into a toilet when it’s half full. You’ll be asked to flush your tubing once a day as well. Make sure you follow the doctor’s instructions regarding the same carefully.
    • Do not go swimming or bathing with your nephrostomy tube on. Prefer using a handheld shower for bathing and prevent your dressing from getting wet.
    • Avoid heavy workouts and strenuous activities. Restrict yourself to light to moderate activity, and increase it gradually.
    • Increase your fluid consumption and maintain a healthy diet.

    Benefits of Nephrostomy

    • The procedure helps maintain kidney function.
    • It prevents potential damage and infections of the kidney.
    • This procedure involves the injection of contrast dye and ultrasound/x-ray scan that can help in diagnosing underlying conditions or abnormalities.
    • It provides access to treat other underlying conditions.

    Risks of Nephrostomy

Every procedure has some recognized risks, and so does nephrostomy. While the procedure is quite safe, we can’t turn a blind eye to the potential risks. Some common risks of this procedure are listed below:

    • Excessive bleeding
    • Blockage of the tube
    • Tube dislodgement
    • High fever caused by severe infection
    • Damages to the surrounding tissues and structures.
    • A vascular injury
    • Allergic reaction to anaesthesia, contrast dye, or other medication involved in the process.

    Frequently Asked Questions on Nephrostomy

What is the duration of the nephrostomy procedure?

The nephrostomy procedure typically takes 30 to 60 minutes. However, the duration of the procedure can differ based on various factors such as the doctor’s experience, the patient’s overall health, the reason for the nephrostomy, etc.

When is the nephrostomy tube removed?

Removal of the nephrostomy tube is completely based on the health of the patient and the underlying condition. The tubes are generally replaced every 8 to 12 weeks. Sometimes, the tube may be removed in a couple of months. In the rest of the cases, they may be kept for years.

When will I be able to go back home after the nephrostomy?

Most patients can go back home on the day of the surgery with some precautions. However, this entirely depends upon the overall health of the patient.

    How painful is nephrostomy?

A nephrostomy procedure is generally quite comfortable as it is performed under local anaesthesia. However, you might experience some bruising and slight pain at the site of the operation. Your doctor will suggest ways to manage the pain. You may also notice some blood in the urine. However, it generally goes away in a day or two.
