    How is otitis media treated?

    Otitis media – also called middle ear infection – is the inflammation or infection of the middle ear that can occur in conjunction with common cold, sore throat, respiratory infection, eardrum perforation, etc. It is generally more common in children as compared to adults, however, it is equally dangerous to both.

    Before treatment, the patient should undergo a thorough diagnosis. Diagnosis for otitis media entails physical examination using an otoscope, followed by hearing tests like audiometry, tympanometry, tuning fork test, etc. Depending on the results of the diagnostic tests, the ENT doctor will determine the best course of treatment.

    The choice of treatment for middle ear infection depends on the patient’s age and health condition, severity of the condition, previous treatments, etc. Generally for mild to moderate cases, conservative treatment through watchful waiting and medications (ear drops, oral antibiotics) is preferred. 

    If the patient has an infectious discharge trapped behind the eardrum that has potential for causing further complications or if the medical treatment is not working, surgical intervention becomes necessary. Surgical intervention for otitis media entails:

    • Myringotomy: During this procedure, the surgeon creates an incision to drain the discharge and then closes the incision and packs the ear canal with dressing and bandages to let the patient heal. It is usually performed when one-time drainage is sufficient to treat the patient, for e.g. blood collection in the middle/inner ear canal after a trauma, etc.
    • Myringotomy with tympanostomy: A tympanostomy is often combined with a myringotomy for severe chronic inner/middle ear infections where repetitive drainage is required. During the surgery, ear tubes are placed through the eardrum to allow for continuous drainage over a long period of time. There are different types of tympanostomy tubes such as T-tubes, grommet tubes, etc., based on their length, flexibility, etc. The type of tube to be inserted is selected by the ENT specialist based on the patient’s symptoms, age, and requirements.

    What to expect after otitis media treatment?

    Mild and acute ear infections generally clear up with antibiotics within 3-5 days, but the patient should complete their antibiotics course to prevent recurrence of the infection. However, for chronic ear infections, surgery becomes necessary which has a longer recovery period. With myringotomy, complete healing usually occurs within 4-5 weeks. However, with tympanostomy, ear tubes generally remain in the ear for around 6-12 months, after which they fall out on their own.

    You can prevent the occurrence and recurrence of otitis media by following the given preventive tips:

    • You should not smoke and avoid exposure to smoke as much as possible as smoke from cigarettes, even second-hand, can increase the risk of ear infections.

    • Hydrogen peroxide has proven to be very effective in fighting off ear infections. Just place a few drops of hydrogen peroxide in your ear for a few minutes and then drain it out. Be careful not to put too much peroxide and to not let it sit too long.
    • If you are prone to allergic reactions, manage your allergy and control all contact with your allergens.
    • Do not insert a Q-tip in your ear whether to clean it or to remove infectious drainage.
