What is an overbite?

An overbite, most commonly known as buck teeth, is the misalignment of the teeth in which the upper front teeth stick out beyond the lower front teeth. It usually occurs due to genetic factors like jaw shape, but can occur due to external factors and oral habits like nail biting, teeth grinding, tongue thrusting, extended pacifier usage, etc.

There are two main types of overbite – skeletal overbite and dental overbite. Skeletal overbite occurs due to irregular jawbone development, causing teeth and jaws to grow improperly. On the other hand, a dental overbite occurs due to external factors that interrupt teeth development, i.e., overcrowding,  improper loss of teeth, poor oral habits, etc.

Skeletal overbite is generally treated through jaw surgery and comprehensive orthodontic therapy, especially in adults where the teeth roots and jaw bones have developed and set firmly. However, for children who are still growing, surgery is rarely performed for overbite correction.

Why is overbite correction necessary?

Overbite, also known as deep bite or closed bite, is very common and often not noticeable in mild cases. Thus, it usually does not cause any noticeable symptoms or difficulties. However, it can result in some cases, it can result in health issues in the future, such as:

  • Discomfort or difficulty while breathing
  • Difficulty or pain while chewing
  • Sleep apnea
  • Gum disease
  • Jaw pain and speech difficulties
  • Temporomandibular disorders (TMD)
  • Tooth decay

In addition to these problems, a moderate to severe overbite can result in poor smile aesthetics and facial appearance, which can result in psychological issues in the patient, such as stress, anxiety, self image issues, depression, etc.

What happens in overbite treatment?

The treatment procedure begins with a thorough diagnosis. Typically, the dentist begins the diagnosis process with an oral examination to assess the health status of the oral health tissues like gums, teeth, etc. They perform the auxiliary treatments like filling, cleaning, root canal treatment, capping, etc., before beginning the treatment process.

Following this, they perform imaging tests like dental x-rays, OPGs, cephalograms, etc., to find out the nature and cause of the malocclusion. For aligner treatment, the orthodontist then performs a 3D scan to fabricate the dental aligner trays.

The patient receives the aligners at-home along with instructions on how to wear them. The treatment can last from 6-24 months, depending on a variety of factors. Following the completion of the treatment, the patient also receives clear retainers they have to wear to retain the effects of the treatment.

What are the benefits of overbite treatment through dental aligners?

There are a variety of benefits of overbite correction treatment through dental aligners, such as:

  • Smile makeover and better aesthetic appearance after the treatment
  • Removable braces require no dietary restrictions
  • Easier to brush, floss, and maintain dental hygiene as aligners can be removed at will
  • No cuts or trauma to the oral soft tissues

Other treatment options for overbite correction

The treatment option differs for children and adults, especially if the child’s jaw is still in the growing stage. During childhood, overbite is resolved using a variety of orthodontic appliances such as:

  • Growth modification devices, such as palate expanders, headgears, bionator, etc., that utilize the growth capability of the jaw bone to expand and reposition the jaw during growth spurts
  • Fixed braces to reposition crooked teeth back to their proper place
  • Removal of any supplementary teeth or remaining deciduous teeth to make room for permanent teeth so they can erupt in proper position
  • Other orthodontic appliances like palatal crib, mouth guards, etc., that help treat poor oral habits

On the other hand, for adults, following treatments are preferred for overbite correction:

  • Clear aligners to reposition the teeth in their proper position
  • Jaw surgery to correct jaw alignment in cases where overbite exists due to jaw misalignment

FAQ

How much overbite is normal?

Generally, an overlap of 1-3 mm. between upper and lower teeth is considered normal, however, overbite larger than this can cause problems and should be treated. If you can see less than 50% of your lower teeth in proper occlusal position, then there is a high chance you have an overbite.

Can I live without receiving any treatment for overbite?

Yes, since overbite is not primarily painful or symptomatic, you can live without receiving treatment for overbite, especially in mild cases (3-4 mm.) However, severe overbite can lead to health issues from teeth grinding to facial asymmetry, headaches to, so it should be treated.

Can overbite correct itself without treatment?

Generally, the answer is no, in fact, untreated overbite can worsen over time. However, in children and toddlers with developing jaws, overbite can be a part of the tooth eruption process. Most orthodontists do not recommend treatment for overbite below the age of 7.

What kind of speech problems can occur due to overbite?

The excessive overlap of upper and lower teeth in an overbite can lead to speech impediments like lisping, slurred speech, clicking sound while moving the jaw, etc.

Which is better for overbite correction - aligners or braces?

Both fixed braces and aligners can reliably correct a  overcrossbite. There are challenges for both treatments, for example metal braces can cause prolonged pain and discomfort, while clear aligners rely on the patient, so if the patient does not wear them for at least 20-22 hours a day, they can fail. Your orthodontist will help you select the best treatment option for you.
