  • What is Stapler Surgery (Stapled Hemorrhoidectomy)?

Stapler surgery or stapled hemorrhoidectomy is a surgical technique for treating piles. It uses a stapling device to remove piles tissues. Doctors recommend this treatment procedure for patients whose piles have prolapsed or become abnormally large or who have shown little or no improvement with nonsurgical treatment.

Under stapler surgery, the pile’s tissue is lifted into a ring of tissues with sutures, and a stapler removes it, cutting off blood flow to the tissue. As a result, patients who undergo stapler surgery for piles typically experience less pain than those who undergo traditional piles surgery. They also experience less bleeding, itching, and swelling around the anus and inside the rectum. In addition, stapler surgery may reduce the chance of incontinence after the procedure.

Diagnosis And Procedure Of Stapler Surgery For Piles

    Diagnosis

If you are experiencing symptoms of piles like extreme itching and pain around the anus, uncomfortable lump, or swelling near the anus, you may need stapler surgery to treat it. To determine that, the surgeon will perform a variety of diagnostic tests like:

Physical exam – Your doctor will inspect the area around the anus for-

    • lumps or swelling near the anus
    • internal piles that have fallen through the anal opening, known as prolapse
    • external piles with a blood clot in a vein
    • leakage of stool or mucus

Digital rectal exam – Your doctor can perform a digital rectal exam to-

    • analyze the tone of the muscles in the anus
    • check for tenderness, blood, and lumps or masses

Anoscopy– An anoscopy is used to diagnose piles. For an anoscopy, the doctor uses a small tube known as an anoscope to view the lining of the rectum and anus. The doctor examines the tissues lining your rectum and anus to look for signals of lower digestive tract problems and swelling or hard lumps around the anus. A related procedure called high-resolution anoscopy uses a particular magnifying device called a colposcope and an anoscope to view the anus and rectum area.

Rigid proctosigmoidoscopy – Rigid proctosigmoidoscopy is similar to an anoscopy, except that here the doctor uses a proctoscope to view and inspect the lining of your rectum and lower colon

    Procedure

In stapler surgery, piles are removed, and a part of the anal region is stapled. This surgery is performed under a locoregional or general anesthetic and takes around 30 minutes.

    • In this procedure, a circular hollow tube is inserted into the anal canal. A long woven suture is placed circumferentially within the anal canal and above the piles through this tube.
    • A stapler is a 33mm disposable instrument with a circular stapling device attached to it. The stapler is placed through the hollow tube, and the ends of the suture are pulled.
    • The doctor drags the suture, which then draws the expanded piles tissues into the jaws of the stapler.
    • Then, the piles cushions are pulled back into their normal position within the anal canal, and the stapler is ejected. It cuts off the circumferential ring of expanded piles tissues which is trapped in the stapler, and at the same time, staples together the upper and lower edges of the cut tissue.
    • Stapler piles surgery stops the blood supply to the piles causing them to shrink.

Are you going through any of these symptoms?

How to Prepare for Stapler Surgery for Piles (Stapled Hemorrhoidectomy)?

Before your stapler surgery for piles –

    • Convey your doctor about all medications you are consuming, including supplements, vitamins, and blood thinners.
    • Do not drink or eat anything after midnight the night before your stapler surgery.
    • Ensure that you have someone to give you a ride home from the hospital after your surgery.
    • Be sure to take your suppository as prescribed by your doctor.
    • You must also undergo a cleansing bowel regimen before the procedure. A typical regimen may include drinking one 10-ounce bottle of magnesium citrate in the afternoon the day before your stapler surgery.

Risks of Stapler Surgery for Piles

Risks and complications are uncommon in stapler surgery for piles. However, there can be certain drawbacks that can be reduced when an experienced proctologist performs the surgery.

Some of the risks of stapler surgery may include the following:

    • Fecal incontinence
    • Recurrence chances (6-8%)
    • Bleeding and infection
    • Inflammation
    • Restraints from the staple line

Risks and complications after stapler surgery have been negligible, with recurrence of prolapse in 1 %, perianal thrombosis in 3 %, and anal stenosis in 2 % of patients.

    Benefits of Stapler Hemorrhoidectomy Surgery

Stapler surgery for piles is an advanced surgical process, and it has many advantages in contrast to traditional piles surgery, such as

    • Less pain
    • Less bleeding
    • Faster recovery
    • Shorter hospital stay
    • Smaller incisions
    • High accuracy

Stapler surgery for piles has no severe complications. Hence it is widely used by many proctologists. Several patients experience an enhanced quality of life after the surgery. However, patients should follow the doctor’s instructions to avoid complications after going home.

Recover After Stapler Surgery for Piles (Stapled Hemorrhoidectomy)

Though stapler surgery is a relatively new surgical method, it has quickly become one of the most popular treatments for piles surgery. This is because stapler surgery has better results and early recovery in piles treatment.

Due to less postoperative pain and high satisfaction among patients, this surgical procedure has been widely accepted by most surgeons. Recovery from stapler hemorrhoidectomy is linked to fewer complications and an easy and quick return to work. In addition, recovery from this surgery is quicker because the procedure removes the abnormally expanded piles supporting tissues that have led the pile to prolapse downward, not the piles themselves.

The recovery period for stapler surgery is associated with less pain compared to traditional piles surgery. Furthermore, recovery is also reported to be more comfortable because of decreased postoperative incidence of urinary retention, bleeding, constipation, fecal impaction, and prolonged wound healing.

Here are some tips to ensure fast healing after stapler surgery for piles:

    • Take a sitz bath- A sitz bath with warm water can help reduce pain, swelling, and discomfort after stapler surgery.
    • Use medication wisely- Your doctor will prescribe certain painkillers after your stapler piles surgery. The prescription should be followed strictly as overuse of painkillers can lead to constipation.
    • Eat right- The patient should follow a well-balanced diet to prevent constipation and enhance healing after the surgery. The diet should be fiber-rich to allow waste to travel smoothly through the digestive system. In addition, the patient must intake around 8-10 glasses of fluids, which should not include soft drinks, tea, coffee, etc.

Is Stapler Surgery better than traditional surgery?

    Stapler Surgery Open Surgery
Less painful process More painful process
Operation is performed above the sensitive area of the anal canal Operational site is the sensitive part of the anal canal (dentate line)
    Quick recovery Recovery takes time
Operation time is less Operation time is more

What is the cost of stapler piles surgery in India?

The cost of stapler piles surgery can be different for each individual. The minimum cost of this surgery is around Rs. 60,000, which can go up to Rs. 65,000. However, the accurate cost of the surgery depends on several factors, including:

    • Fees of the surgeon
    • Fees of the anesthetist
    • Cost of anesthesia
    • Location of the hospital or the city where the surgery takes place
    • Type of hospital (government/private)
    • Cost of preoperative tests
    • Costs of medicines

Delaying treatment for piles can cause complications that might not be severe but discomforting. Piles, if not treated in time, can lead to issues such as:

    • Thrombosis of piles
    • Rectum prolapse.

    Effectiveness of stapler piles surgery

Then they fall off and pass through the stool unnoticed after several weeks. There is a significant improvement in the condition. Piles adversely affect the quality of life depending on the degree of piles and their symptoms.
