What are Strangulated Piles?

Strangulated piles (strangulated hemorrhoids), is a medical condition where an internal hemorrhoid prolapses (i.e., protrudes outside the anus) and becomes trapped outside the anal sphincter. The blood supply to the hemorrhoid is cut off, resulting in a painful and potentially serious condition.

Strangulated piles are considered a medical emergency and require immediate treatment. If left untreated, the condition can lead to tissue damage, necrosis (i.e., death of tissue), and even sepsis (i.e., a life-threatening infection).

Symptoms of Strangulated Piles

The symptoms of strangulated piles can be quite uncomfortable and may include:

  • Pain and discomfort in the anus or rectal area.
  • Swelling and inflammation around the anus.
  • Difficulty passing stool or feeling like you need to strain during bowel movements.
  • Bright red blood in the stool or on the toilet paper after wiping.
  • Itching, irritation, and discomfort around the anus.
  • A lump or bulge protruding from the anus.
  • A feeling of fullness or pressure in the rectum.

If you experience any of these symptoms, it’s important to consult an anorectal surgeon for a proper diagnosis and treatment. Strangulated piles are a serious medical condition and require prompt medical attention.

Experiencing Any Of These Piles Symptoms?

Treatment Options for Strangulated Piles

Treatment for strangulated piles typically involves a surgical intervention to remove hemorrhoids and alleviate the pressure on the affected area. In some cases, medication may be prescribed to manage pain and reduce inflammation.

The techniques used for the treatment of strangulated piles include the following-

  • Hemorrhoidectomy or Hemorrhoid Removal- It is one of the safest and most effective methods to treat strangulated piles. This technique involves the removal of the excess tissues that cause bleeding. The surgery is performed under local, regional, or general anesthesia. It is an invasive procedure, due to which the recovery takes around 3-6 weeks.
  • Stapled Hemorrhoidectomy Also referred to as hemorrhoid stapling or stapled hemorrhoidopexy, this surgical technique is also a good option for strangulated piles. This method involves blocking the blood flow to the hemorrhoidal tissues. The tissues where the blood supply is already compromised, are removed safely. Then a special stapling device is used to reposition the tissues. Compared to hemorrhoidectomy, it is a slightly less invasive technique that allows faster recovery.
  • Doppler-guided Hemorrhoidal Artery Ligation– This procedure involves using a special tool to identify the blood vessels supplying hemorrhoids and then tying them off to cut off the blood supply. Only then the strangulated tissues are removed. This surgery is carried out after inducing monitored anesthesia care (MAC) that makes the patient fall asleep.

Many people request doctors to use the minimally-invasive laser technique for piles treatment. However, laser piles surgery is not a suitable technique to treat strangulated piles.

How Is Strangulated Piles Surgery Performed?

The specific steps of strangulated piles surgery may vary depending on the surgical procedure used and the patient’s individual case. However, the general steps for a hemorrhoidectomy, which is the most common surgical procedure used to treat strangulated piles, may include:

  • The patient is given anesthesia to numb the area that will be treated. General, local, or regional anesthesia may be used for the procedure. The patient’s vitals are also checked before the surgery.
  • The patient is asked to lie down on the operating table.
  • An incision around the hemorrhoid is made by the surgeon to access the strangulated tissues.
  • Either a scalpel or surgical scissor is used to remove the strangulated tissues and the rest of the hemorrhoid tissues that are inflamed.
  • The incision may be closed with stitches or left open, depending on the technique being used.

After the surgery, the patient will be monitored for a short period in the recovery room before being discharged. Pain medication and stool softeners may be prescribed to manage pain and prevent constipation. A follow-up schedule and recovery guide are also given to the patient to monitor his/her recovery.

Recovery After Strangulated Piles Treatment

The recovery after strangulated piles treatment may vary depending on the type of treatment and the individual patient’s healing process. Here are some general tips that the patient should follow for a smoother recovery-

  • The patient needs to take the prescribed medications as directed to manage the pain and other potential complications, such as infection.
  • The doctor will provide detailed instructions for post-surgery wound care, including how to clean the wound and change dressings.
  • The patient needs to keep the treated area clean and dry and maintain hygiene in the area.
  • It’s important to avoid certain things that can cause irritation in the treated area. This includes performing strenuous activities, such as heavy lifting, exercising, cycling, etc.
  • The patient should make dietary changes and increase fiber intake. Drinking plenty of water throughout the day is also essential to prevent dehydration of the body, which increases the risk of constipation. In some cases, the doctor may also prescribe the use of stool softeners or laxatives.
  • It’s also important for the patient to take proper rest after the surgery and resume walking to promote blood circulation throughout the body.
  • Take regular follow-ups with the doctor to monitor the recovery process and determine if there are any signs of complications.

The recovery duration varies for each patient, depending on the technique used. Patients who have undergone surgery for strangulated piles are likely to need around one week for proper rest and resume activities. Throughout the recovery, it’s crucial to follow the doctor’s instructions to prevent the recurrence of the disease.

Additional Measures to Manage Strangulated Piles

Strangulated piles are considered a medical emergency and require immediate treatment. The treatment options may depend on the severity of the condition and may include:

  • Medications: Over-the-counter pain relievers, such as acetaminophen or ibuprofen, may be prescribed to manage pain and reduce inflammation. In some cases, topical creams or ointments containing hydrocortisone or lidocaine may also be prescribed to provide relief from pain and itching.
  • Sitz baths: A sitz bath involves soaking the affected area in warm water for 10-15 minutes several times a day. This can help reduce swelling and discomfort.
  • Lifestyle changes: Making certain lifestyle changes, such as increasing your fiber intake, drinking plenty of water, and exercising regularly, can help prevent the development of piles.

If strangulated hemorrhoids are not treated on time, it can lead to tissue damage, necrosis, or sepsis. Thus, it’s important to consult an expert anorectal surgeon and take the necessary measures to manage the condition.
