  • What Are Thrombosed Piles?

Piles, also known as hemorrhoids, arise from abnormal distension of the hemorrhoidal veins that are present beneath the mucosa of the lower rectum and anus. There are two kinds of piles – External and Internal Piles.

    • External piles form on the verge of the anal canal, below the dentate line, and are the most commonly occurring type. Thrombosed external piles appear as a painful and discomforting dark blue lump at the verge of the anal canal.
    • Internal piles form in the lower rectum above the dentate line. Thrombosed internal piles are far less common and may not be visible outside unless they protrude from the anal canal.

Thrombosed piles happen when either an external or internal pile fills with blood clots. Thrombosed piles are not dangerous, but they can be extremely painful. If symptoms do not go away, there are numerous treatment options ranging from topical creams to surgery.

    Causes Of Thrombosed Piles

Some possible causes of thrombosed piles include:

    • Pregnancy, from pressure caused by the baby
    • Childbirth, from pushing at the time of delivery
    • Physical exertion caused by repeated heavy lifting
    • Prolonged sitting
    • Constipation, that can lead to straining on the toilet
    • Diarrhea or loose stools
    • Obesity, or having an excessive amount of fat on the body
    • Anal intercourse
    • Not using the toilet regularly

Are you going through any of these symptoms?

    Symptoms Of Thrombosed Piles

Thrombosed piles can be very painful. If you have one, walking, sitting, or sitting at the toilet seat can hurt. Other thrombosed pile symptoms include:

    • itching around the anus
    • bleeding when you have a bowel movement
    • lump or swelling around the anus

Diagnosis and Surgery For Thrombosed Piles

    Diagnosis

An experienced proctologist can easily and effectively diagnose thrombosed piles. Diagnosis of thrombosed piles can be done using clinical evaluation and procedural diagnosis.

    Clinical evaluation

A proctologist conducts clinical evaluation to-

    • Know about the history of severe pain in the anal area.
    • Detect a thrombosed piles by physical examination of a painful lump or inflammation of the perianal tissues.

    Procedural diagnosis

    • Your doctor can perform a digital rectal exam to check for blood in the stool and other issues in the anal and rectal regions.
    • If the digital rectal exam is inconclusive, the anal canal can be analyzed with the help of an anoscope, which is a speculum that can be used to see and inspect the lower rectum.
    • If there is proper clinical evidence of rectal bleeding, a colonoscopy or sigmoidoscopy can be performed by your doctor to know other causes of bleeding, like inflammatory bowel disorder, colorectal polyps, or rectal cancer.

    Surgery

A thrombectomy is usually performed to treat thrombosed piles. Thrombectomy is a minor procedure in which the doctor makes a cut in the inflamed piles and drains the blood. A doctor advises thrombectomy for external piles if you have a huge clot. Or, they may recommend surgery if the blood clot is causing intense tissue injury.

The doctor gives the patient anesthesia before they begin the procedure, so they will not feel any pain. Thrombectomy is most effective if performed a few days after blood clots form in a pile.

If a thrombectomy is not effective, surgery may be needed. There are a variety of surgical options available to treat thrombosed piles. These include:

    • Stapled hemorrhoidectomy: It is a procedure where the piles are stapled in place while a person is under general anesthetic.
    • Laser hemorrhoidectomy Laser hemorrhoidectomy is a new innovative method of hemorrhoidectomy. In this hemorrhoidectomy procedure, a laser will be inserted into the anal canal to eliminate the terminal branches of superior and inflamed pile arteries. The patient will have no surgical incision scar on the anus in this surgery.

    How To Prevent Thrombosed Piles?

It is not always viable to prevent thrombosed piles, but the below-mentioned tips can certainly reduce the risk:

    • Eating high-fiber foods: A high-fiber diet helps in keeping bowel movements normal and regular. Some high-fiber food items include broccoli, whole-wheat pasta, oatmeal, etc.
    • Being more active: Avoiding sitting for stretched periods and walking around every hour can help prevent thrombosed piles.
    • Not straining if constipated: It is always better to take a stool softener rather than push excessively if a person feels constipated.
    • Drinking plenty of water: Being hydrated can help reduce constipation.

What Are The Risks Of Thrombectomy?

All surgery has certain risks and complications. The risks of thrombectomy include:

    • Excess bleeding
    • Infection
    • Injury to the blood vessel at the site of the blood clot
    • Reaction to anesthesia
    • Pulmonary embolism- It is a sudden blockage in a lung artery. It happens when a blood clot breaks loose and travels through the bloodstream to the lungs.

There is also a risk that the blood clot will develop again. Your risks may vary depending on your overall health, how long you have had the clot, and where it is in the body. Consult with your doctor about all your problems and questions.

Post-Operative Care After The Surgery For Thrombosed Piles

Some of the ways to ease the pain and discomfort after thrombosed piles surgery include:

    • Taking over-the-counter painkillers can reduce pain after surgery.
    • Soaking the surgical site in warm water several times daily and gently patting it dry can help reduce pain and discomfort after your surgery.
    • Using an ice pack or a cold compress to the surgical site may relieve pain and swelling after the surgery.
    • Applying witch hazel can also reduce itching and pain at the surgical site.
    • Using wet wipes rather than toilet paper can reduce friction and cause less irritation in the affected area.
    • Aloe vera is known for its anti-inflammatory properties. Applying aloe vera to the surgical site can decrease inflammation.
    • A person should avoid wearing tight clothes made of artificial fabrics after surgery for thrombosed piles. Wearing loose cotton clothing can ease irritation in the surgical area and keep it dry.

    Common FAQs Around Thrombosed Hemorrhoids

Does thrombosed piles go away on its own?

In most cases, thrombosed piles go away on their own after certain weeks, even without treatment. The severity of pain is extreme in the first day or two from the onset. After that, the blood clot will get reabsorbed, and the pain will subside within the next few days. Resolved thrombosed piles can sometimes leave behind some residual skin tag due to excessive skin stretching around the anus.

    Are thrombosed piles serious?

Thrombosed piles are not dangerous but can be extremely painful and lead to rectal bleeding if they become ulcerated.

Can I drain the thrombosed pile myself?

Surgical removal or drainage of the blood clot is a procedure that can have significant risks, like incomplete removal of the clot, unmanageable bleeding, and infection of the perianal tissues, which is why it should always be performed by a trained and experienced proctologist.

    Do thrombosed piles burst?

If a blood clot forms inside the piles, it can become thrombosed and eventually burst when the internal pressure rises (during excessive straining from either diarrhea or constipation).

    Are thrombosed piles cancerous?

No. Thrombosed piles do not lead to cancer.

Are thrombosed piles hard to touch?

Thrombosed piles happen when blood pools in external piles, forming a hard and painful lump. This lump can be extremely hard to touch.

Is coconut water good for thrombosed piles?

One of the best ways to prevent thrombosed piles is to hydrate yourself. Drink plenty of water, fruit juices, or coconut water throughout the day. It will help keep the stools soft so that it passes out without causing much pain.
