What Is Prostatitis?

Prostatitis is a discomforting and often painful condition which causes the prostate gland to become inflamed, swollen, and tender. The condition can affect men of all ages, and causes different symptoms including issues with urination such as difficulty urinating, urgency to urinate, blood or cloudiness in the urine, in addition to pain in the abdomen, groin, or lower back, discomfort in the penis or testicles, fever, chills, etc.

The prostate gland is a walnut-sized gland located below the bladder in men, which, along with other glands, produces fluids responsible for transporting sperm during ejaculation. Prostatitis is divided into different types, and the causes and symptoms of each type vary. Different types of prostatitis include:

  • Chronic pelvic pain syndrome or CPPS: This is the most common type of prostatitis. It is distinguished by chronic pain in the pelvis, perineum i.e., the area between the scrotum and rectum, and the genitals. This type of prostatitis affects 1 out of 3 men.
  • Acute bacterial prostatitis: This type of prostatitis is caused by a UTI that triggers an infection in the prostate gland. Common symptoms of acute bacterial prostatitis include fever, chills, painful urination, or issues urinating. This condition requires immediate medical intervention, or may aggravate.
  • Chronic bacterial prostatitis: This condition is caused due to bacteria trapped in the prostate gland. It can flare up UTIs, and is quite difficult to treat.
  • Asymptomatic inflammatory prostatitis: This type of prostatitis causes prostate gland inflammation without exhibiting any symptoms. This condition is often diagnosed during diagnostic tests for other conditions. Generally, it does not require treatment.

Treatment

Diagnosis

Accurate diagnosis is critical to ensuring effective treatment for prostatitis. Therefore, before laying out a treatment plan for you, the doctor will perform a thorough diagnosis to determine the severity of the condition and rule out other complications that might cause hindrances in the treatment. Some standard diagnostic tests performed before prostatitis treatment include:

Physical examination: During the initial physical examination, the doctor will ask you questions about the symptoms you’re experiencing, your medical history, and so on. They may also ask you about your lifestyle habits and the medicines you’re taking. Make sure you answer all the questions correctly.

Digital rectal examination: In this test, the doctor will insert a gloved and lubricated finger into your rectum to assess your prostate gland. They will look for swelling or pain in the area through this examination. They may also massage the prostate to collect seminal fluid for further assessment.

Urinalysis: This test involves urine culture and urinalysis to look for bacteria and urinary tract infections.

Blood tests: A blood test is performed to check the levels of Prostate-specific antigen or PSA, which is a protein made by the prostate gland. High levels of PSA may hint at prostatitis, BPH, or even prostate cancer.

Transrectal ultrasound: Men diagnosed with acute bacterial prostatitis or chronic prostatitis that doesn’t respond to antibiotics might be asked to undergo a transrectal ultrasound. During this exam, a thin ultrasound probe is inserted into the rectum. It uses sound waves to produce images of the prostate gland, and allows doctors to look for abnormalities in the prostate gland, stones, or abscesses.

Cystoscopy: In this test, the doctors use a cystoscope, i.e., a pencil-sized tube with a camera or viewing lens, to get a clear view of the bladder and urethra. This test may not help in the diagnosis of prostatitis but may help rule out other urinary tract conditions.

Treatment For Prostatitis

Conservative Treatment

After performing a thorough diagnosis and assessing the findings, the doctor will suggest the best treatment plan for you. This may include treatment through conservative or non-conservative methods. Also, there’s no one-size-fits-all treatment for prostatitis- it varies based on the type of prostatitis, the severity of the condition, and so on. Some common treatments for prostatitis are as follows:

  • Antibiotics: If bacteria are responsible for causing severe prostatitis symptoms, your doctor will most likely suggest antibiotic treatment through an IV at first. Following this, you may be asked to take oral medicines for some time. The course of treatment will depend upon how severe your symptoms are.
  • Alpha-blockers: These medicines help relax parts of the urinary tract, and reduce obstruction from an enlarged prostate. These conditions are generally responsible for UTI and prostatitis.
  • Nonsteroidal anti-inflammatory drugs: Pain-relief medicines may be prescribed to help relieve soreness and swelling caused by prostatitis. Also, antidepressants or anti-seizure medicines may be suggested for treating long-term prostate pain or discomfort.
  • Pain medicine: Antidepressants or anti-seizure medications are two types of medicine that can be used to treat long-term prostate pain.
  • Physical therapy: In some cases, prostatitis can occur as a result of issues with your pelvic floor muscles. These muscles support your bladder and bowel and even help with sexual function. The doctor may refer you to a physical therapist to enhance your posture and strength.
  • Prostate massage: A prostate massage can help drain fluid from the prostate ducts or tubes. Patients who get it two to three times weekly can notice a relief in the symptoms.

Non-Conservative/Surgical Treatment

Transurethral Resection of the Prostate (TURP)

In general, prostatitis does not require surgery and can be managed through conservative methods. However, in rare cases, a surgery called Transurethral Resection of the Prostate or TURP may be suggested. In this surgery, a section of the prostate is cut, which helps in relieving the symptoms.

This surgery is only suggested for older men, as it may cause retrograde ejaculation and may lead to sterility, incontinence, and impotence.

  • Avoiding alcohol, caffeine, and spicy food, which can flare up an irritation of your bladder.
  • Drinking ample water throughout the day. This will help flush bacteria out of your bladder and relieve symptoms.
  • Do not sit on a desk or chair for prolonged periods. Prefer sitting on a cushion.

What questions should I ask the urologist during my appointment?

During your consultation with the urologist for prostatitis, make sure you ask them the following questions:

  • What may have caused the condition?
  • What type of prostatitis do I have?
  • What will the diagnosis procedure consist of?
  • How severe is my condition?
  • What are the risks and side effects of treatment?
  • Will I require surgical treatment for prostatitis?
  • What are the signs of prostatitis complications?
  • How do I prevent the condition from recurring?
  • Do I need to avoid sex during/after prostatitis treatment?

Is prostatitis recurrent?

Yes. Men who have had prostatitis can have it again in the future. A recurring infection can be treated with antibiotics. Recurrent infections generally occur when the antibiotics aren’t able to go deep into the prostate tissue to kill all the bacteria.

Is surgery always needed for prostatitis?

No. Most cases of prostatitis are manageable through conservative methods, i.e., medicine, therapy, and lifestyle modifications. Surgery is suggested in very rare cases when conservative methods fail to relieve the symptoms.

Is prostatitis treatment covered under insurance?
