    What Happens In Pyeloplasty?

    Diagnosis

    The following diagnostic tests are standard before Pyeloplasty: 

    • Physical Examination: This is the first step of the diagnostic process before Pyeloplasty. The urologist will question you about the symptoms you’re experiencing and your medical history. If they suspect UPJ, they might suggest some other tests. 
    • Ultrasound: An ultrasound shows a dilated renal pelvis with a collapsed proximal ureter and higher resistive indices due to the blocked kidneys. However, an ultrasound is not enough to confirm UPJ. 
    • Blood Samples: Several blood tests, including Blood Urea Nitrogen (BUN) and creatinine tests, are performed to check if the kidneys are functioning properly. 
    • An intravenous pyelogram (IVP): In this test, a dye is injected into the bloodstream, after which an x-ray is performed to see how the kidney filters the dye from the blood. As the dye passes through urine, the doctors can see how the kidney, renal pelvis, and ureter appear and determine abnormalities. 
    • A nuclear renal scan: This test is quite similar to IVP. However, in this test, radioactive material is used instead of dye. The doctors monitor the material with a special camera, check the amount of blockage, and determine the overall function of the kidneys. 
    • CT scans: A CT scan allows doctors to see an obstructed kidney easily. It is generally performed in emergency cases of severe pain.

    Treatment

    Pyeloplasty is performed in severe cases of UPJ when poor drainage fails to get better on its own, and the condition becomes worse. There are two types of pyeloplasty surgeries: 

    Open Surgery: This is the conventional pyeloplasty technique. This method removes the blockage, and a wide opening is created by reattaching the renal pelvis and the ureter. This allows quick and easy urine drainage and even reduces the risk of infections. It is performed in the following steps:

    • The patient is sedated with general anesthesia. 
    • The surgeon makes a 2 to 3-inch long cut right below the ribs. 
    • The obstructed part of the ureter is extracted, and the normal caliber ureter is attached to the renal pelvis.
    • Finally, a stent, a small silicone tube, is placed to help drain urine from the kidney. 
    • The pyeloplasty stent removal procedure is performed once the surgical area is healed. 

    In case of open surgery, the patient (adult or baby) may have to stay in the hospital for a day or two.  

    Laparoscopic Pyeloplasty Surgery: This is the modern approach to performing pyeloplasty surgery, which is also minimally invasive and causes minimal discomfort to the patient. In this method, a small camera or scope is used, along with some surgical tools, to perform the surgery. Laparoscopic Pyeloplasty Surgery is performed in the following steps:

    • The patient is asked to remain in the hospital for a day or two after surgery. They will be allowed to eat something and consume fluids within a couple of hours of the procedure.
